Position Details
Position Title:Community Support Worker
Reports to:Service Manager
Date:June 2012
Purpose
  • To maximise the independence of the people we support, by providing quality services to the person, their families and carers in a person centred approach.
  • Uphold the Rights and responsibilities of the people we support in line with Human rights, the Disability Services Act and Sunnyfield policy and procedures (CHCCS301A Work within a Legal Framework).
  • Advocate for the people we support where appropriate (CHCAD1C Advocate for Clients).
  • To perform or assist in a Key Worker role as required.

Environment
  • Community services have a range of programs operating at varying hours over seven days.
  • The position requires a degree of flexibility to adapt to different working environments (e.g. service site, in the community or the person’s home).
  • Actual hours of work will vary depending upon your place of employment and conditions of employment contract.
  • The position requires driving and assisting people with transport between service sites and community facilities.
  • The position may require lifting and repetitive work.
  • This position will be based regionally and will involve travel to all Sunnyfield sites and other locations as required.

Position Scope
  • To ensure that the people we support are being supported to achieve what is important to them, through maximising their independence.
  • To provide quality services to people with disabilities, their families and carers.

Reporting Relationships
  • Team Leader.
  • Service Manager.

Knowledge, Skills, Qualifications & Experience Required
Qualifications
  • Current Class C NSW Drivers Licence.
  • Senior First Aid Certificate (desirable).
  • Other relevant qualifications (desirable) i.e. Welfare, Certificate IV in Disability Studies or a willingness to obtain.

Relevant Experience
  • Minimum 12 months experience in the disability sector or related field (desirable).

Knowledge & Skills
  • Demonstrated knowledge of the Disability Service Act and associated Standards.
  • Strong communication and interpersonal skills.
  • Well developed planning and organisational skills.
  • Knowledge of OH&S principles.
  • Language from another country where required.

  1. Person Centred Practices
Assist in the development, implementation and review of Essential Lifestyle Plans (ELPs) and Individual Futures Plans (IFPs) for the people we support in line with current policy and procedures.
To develop and review their One Page Profiles.
  1. Support
Assist people we support with their personal care needs as required i.e. mealtime, toileting, personal hygiene, medication etc.
To work with the people we support, in line with keynotes, all relevant plans and Sunnyfield Policies, in a professional manner
Where required assist the people we support to plan and record their personal daily spending in line with relevant organisational policy and procedure
To provide an inclusive and appropriate services to people we support from an Aboriginal and/or Torres Strait Islander background and a Culturally and Linguistically Diverse background
  1. Community Integration and Participation
To assist the people we support in making decisions in regards to their chosen activities.
To assist the people we support to participate and access community facilities/events.
To encourage the people we support to develop and maintain family, friend and community relationships.
  1. Maintain the Health and Safety of the People We Support
Provide assistance to the people we support to maintain their health including following medication regimes, health plans and other medical documentation.
To encourage the people we support to maintain a healthy lifestyle and well being.
For staff to always take on positive approach to behaviour implementing a persons behaviour support plan.
  1. OH&S Responsibilities
Carry out duties in a manner which does not affect your own health or the health and safety of others.
Cooperate with measures introduced in the interests of workplace health and safety.
Undertake all training in relation to OH&S.
Report all OH&S matters to the supervisor and/or OH&S committee representative.
Wear all personal protective equipment provided.
Follow all safe working practices.
Only perform tasks that you are trained and competent to perform.
To monitor and implement changes, as instructed, to achieve a safe working environment for staff, People We Support and visitors to the outlet/workplace.
To implement Incident Reporting procedures.
  1. Team and Administration Requirements
To update the people we support records and routines as necessary.
To record and monitor all financial transactions
as per relevant policies and procedures.
To ensure that any relevant information /changes regarding service delivery for the people we support, is documented to the Service Manager and others as required.
To complete all required documentation within required timeframes and also in a legible, objective and concise manner.
To be professional and be able to work effectively:
  • as part of a team
  • unsupervised
  • at times in isolation
To participate in regular supervision meetings with the Service Manager.
To assist in the day to day ongoing upkeep i.e. cleanliness, tidiness of the site and vehicles.
On an annual basis to participate in a Performance Appraisal Review.
To attend all relevant staff, key worker and People We Support meetings, forums and training as directed by the Service Manager.
  1. Other Duties & Responsibilities
To be involved in the implementation of quality improvement strategies for the service delivery.
