2012 Community Impact Grant Application

Thank you for partnering with United Way of Southeast Alaska to help strengthen the communities in our region. United Way of Southeast Alaska has two grant programs this year.

A)  Learn United: Is to increase the number of children who can read at grade level by the end third grade. Recognizing that there are many barriers and factors that lead to or prohibit success, United Way of Southeast Alaska has chosen to mobilize our communities to focus on literacy by third grade, a critical skill for success in school, work and life.

B)  Live United: Is to address any other identified need in the areas of Health, Education and Income Stability within the community through a matching grant.

PART 1: GENERAL INFORMATION

A. Learn United: GRANT FUNDS FOR PROGRAMS THAT LEAD TO LITERACY BY THIRD GRADE

Funds must to be used within the twelve months following the award date. The entire grant amount can either be awarded to one program or shared among more than one program depending on the committee’s scoring of proposals.

B. Live United: MATCHING GRANT FUNDS FOR PROGRAMS THAT ADDRESS OTHER IDENTIFIED COMMUNITY NEEDS

A second grant program sets aside funds for matching grants for any additional social service needs other than literacy. Special attention will be given to grants that address documented needs in a community (or in communities) in which the project is located. Funds must to be used within the twelve months following the award date. Individual awards will generally not exceed $1500, but will ultimately be at the discretion of the United Way Community Impact Committee.

C. FUND RESTRICTIONS

Funds cannot be used to pay for overhead cost such as rent or utilities. Funds may be used to pay salaries, purchase materials, or pay for outreach.

D. APPLICANT QUALIFICATIONS

Applicants for these funds must have 501(c)(3) status or be a not-for-profit entity, clearly demonstrate fiscal integrity, have a local board, provide evidence of outcomes experience, and demonstrate the capacity to carry out the intended programs.

PART 2: PROPOSAL REQUIREMENTS

A. Learn United

Applicants must clearly and concisely describe their program, including materials to be used, staff experience and training in the literacy field, location of programs and outreach efforts. It should show how the program is tied to children’s ability to master reading in English proficiently by the end of their third grade year of education. You may attach copies of materials and research of program techniques if appropriate. Agencies are encouraged to collaborate with others in the development of the program. This section should be limited to no more than two pages based on approximately 300 words per page.

B.  Live United

The matching grant will rely on the nonprofit’s ability to show where other funding will be coming from and how the requested amount fulfills the project need. In-kind donations will not be counted towards a match in this case although in-kind may be used to show the total impact of a project’s budget. Please indicate what documented need this project or program helps to fulfill in the community served. This section should be limited to no more than two pages based on approximately 300 words per page.

C.  AGENCY CAPACITY

Applicants must clearly demonstrate their capacity to implement their proposed program. If staff is not in place to perform the proposed functions, the applicant must provide an organizational chart, action plan, and timetable for implementing any and all proposed services. This section should be limited to no more than one page based on approximately 300 words per page.

D. WORKPLAN

Your proposed work plan should describe the program you will be conducting with these funds. This section is limited to no more than 2 pages based on approximately 300 words per page and must include the following:

1.  Provide a detailed description of the program you are proposing.

2.  If applying for Learn United Grant Program (literacy) identify how your program will address the need for early literacy training and proficiency. If applying for the matching Live United Grant Program, identify the need and matching funds for the project or program.

3.  Identify the geographical area(s) of the population to be served.

4.  Identify any other agencies, community initiatives, coalitions, etc. that you will partner or work with to accomplish your goal. Please include a letter of support from your partner(s).

5.  Describe how the proposed program complements your agency’s current program structure or mission and promotes volunteerism.

6.  Describe how you will evaluate the program’s effectiveness.

7.  Define two program outcomes and how they will be measured.

8. Describe how you will acknowledge and promote United Way of Southeast Alaska’s contribution to this program.

E.  BUDGET

Using the attached budget form (Attachment 1), provide a detailed budget for your program. This budget should include the total cost, itemized by relevant expense categories. All applicants must attach a one page budget narrative based on approximately 300 words per page.

F.  INTERVIEW

At the discretion of the committee members, an agency representative may be asked to meet with them to answer questions about your proposal and/or provide additional information. A mutually agreeable time and date will be arranged if such a meeting is requested by the committee.

G.  REPORTING

A written one page progress report will be expected on or before September 30, 2012. Then you will be asked to present to the United Way of Southeast Alaska Board either in person or via teleconference mid-year during the months of October, November, December or January. Within 30 days of the end of your project (11 months from award date), you will be expected to submit a final written report. The final report should provide information on the project, including number of participants, outcomes, use of resources, funds expended, as well as demographic information on participants including age, race, gender, handicapped status, income and ethnicity. Call or write to obtain a date for the mid-year presentation. The UWSEAK BOD meets the third Thursday evening of each month at 5:15pm.

H.  OTHER REQUIRED DOCUMENTATION

Applications from any agency that are not currently Partner Agencies of United Way of Southeast or are not receiving funding directly from, or through, the United Way of Southeast Alaska must also include the following:

·  Internal Revenue Service letter of 501 (c) (3) tax exemption

·  By-laws

·  Names and addresses of board members and officers

·  Most recently completed audit and IRS form 990

·  Agency mission statement, goals and objectives.

I.  Publicity and Recognition

All grants received will require the accepting nonprofit to attach or include United Way of Southeast Alaska’s name or logo to any press release, printed material, advertising or other promotion of the project or program. Please indicate how you will publically recognize the contribution made by United Way of Southeast Alaska to your project. A logo will be provided to you for your use.

Five copies of the proposal and budget should be returned to the Community Impact Committee at United Way of Southeast Alaska, 3225 Hospital Drive #201, Juneau, Alaska 99801 by mail or in person by 4:00 PM on April 30, 2012. No time extensions will be granted and incomplete applications will not be considered.
