Introduction

Thank you for using the West of England Academy Education Pathway to develop your understanding of Quality Improvement (QI) Science.

This passport is designed to allow you to evidence your achievement in gaining QI knowledge and skills, and demonstrating their application.

Remember:

The Academy training is recognised by healthcare and academic organisations across the region. Please visit our websiteto see a list of all member organisations.

As you complete each stage of the pathway, please submit your passport to the West of England Academy team for endorsement. The team will guide you on how to progress to Step Two and Three.

To evidence completion of training please get you line manager to sign against the modules completed below, whether you undertake the Academy training or have covered the training elsewhere. Once signed, please scan and send to . The Academy team will endorse this training and return your stamped passport and details of how to progress to Step Two.
