DUNNELLON HIGH SCHOOL

THE FOLLOWING MUST BE PROVIDED FOR REGISTRATION AT THIS SCHOOL

NEW REGISTRATION FOR STUDENT ENROLLING FROM OUT OF STATE

1.  A parent or legal guardian must accompany each student registering to attend Dunnellon High School. The signature of a parent or legal guardian is required on all of the enrollment forms. (Unless the student is 18 years old.) Parent ID is required.

2.  A parent or legal guardian must provide valid proof of residence that the parent/legal guardian reside in the Dunnellon High School district or have an approved Out of Area Request. Please provide an electric bill, utility bill, rental agreement, purchase agreement for this verification. (A driver’s license is NOT acceptable.)

3.  A parent or legal guardian must provide a withdrawal form from the previously attended school with withdrawal grades and/or a report card grade. At the first of a new school year this must be final transcripts or the final report card from the previous school year.

4.  A parent or legal guardian must provide a copy of the student(s) birth certificate.

5.  A parent or legal guardian must provide a copy of the student(s) social security card/number.

6.  A parent or legal guardian must provide a Florida Certificate of Immunization Certificate (Blue Card form DH680) – Students who do not have a Florida Certificate of Immunization (HRS DH680 Blue Card), will have to take their shot records to one of the Health Departments listed below for completion of the DH680 form. Parents will have 30 days to return the Immunization Certificate to the school. The student will not be allowed to attend school without this certification.

7.  Students registering from an out of state school are required to have a physical BEFORE entering a Florida school. The parent may present a record of a physical exam given within the last calendar year stating the student is in good health. If the parent is unable to provide a record of a physical examination the student will be required to provide proof of a physical from a Florida physician.

NEW REGISTRATION FOR A STUDENT FROM ANOTHER FLORIDA SCHOOL

1.  A parent or legal guardian must accompany each student registering to attend Dunnellon High School. The signature of a parent or legal guardian is required on all of the enrollment forms. (Unless the student is 18 years old.) Parent ID is required.

2.  A parent or legal guardian must provide valid proof of residence that the parent/legal guardian reside in the Dunnellon High School district or have approved Out of Area Request. Please provide an electric bill, utility bill, rental agreement or purchase agreement for that verification. ( A drivers license is NOT acceptable.)

3.  A parent or legal guardian must provide a withdrawal form from the previously attended school with withdrawal grades and/or a report card. If it is the beginning of the new school year you must provide final transcripts or report card from the previous school year.

4.  A parent or legal guardian must provide a copy of the student(s) birth certificate.

5.  A parent or legal guardian must provide a copy of the student(s) social security card/number.

7.  No physical is required if the student has ever attended a Florida school, public or private as long as verification of this provided. (located on student’s transcripts or copy of the physical form)

NEW REGISTRATION FOR IN COUNTY TRANSFERS

1.  A Parent or legal guardian must accompany each student registering to attend Dunnellon High School. The signature of the parent or legal guardian is required on each enrollment form. (Unless the student is 18 years old) Parent ID is required.

2.  A parent or legal guardian must provide valid proof of residence the parent/legal guardian reside in the Dunnellon High School district or have an approved Out of Area Request. Please provide an electric bill, utility bill, rental agreement or purchase agreement. If you are living with a relative or friend that resides in the Dunnellon High School district you must provide an electricity disconnect notice from your previous residence and also have the owner of the residence you are residing with complete a Notification of Physical Residence Form. (Form is provided to you in the Guidance Office.)

3.  If the school year is in progress the parent or legal guardian must provide a withdrawal form from the previous school attended in the Marion County School District.
