Application for inclusion of a qualification

in the List of Quotable Qualifications

1.  It is the duty of the applicant to provide sufficient information to support the application. The Secretariat will not assist in obtaining the necessary information from either the applicant or the conferring institution.

2. The completed application form should be submitted together with the following documents –

(a)  a copy of the certificate or other documentary evidence showing the award of the qualification to the applicant; and

(b)  curriculum or course prospectus of the programme showing details on the content and duration of the programme and the level of the qualification.

3. The application will be considered on the basis of the information provided by the applicant. If there is insufficient information for the Physiotherapists Board to properly assess the qualification, the application will be rejected.

5.  Reference may be made to the Guidelines for Consideration of Quotable Qualifications which is available from the website of the Physiotherapists Board at http://www.smp-council.org.hk/pt/english/index_lqq.htm .

6.  Application must be submitted by a registered physiotherapist in possession of the qualification under application.

7.  Application form and relevant documents should be submitted to the Secretariat of Physiotherapists Board via one of the following means –
