Terms of Reference - Integrated Care Partnership Committee

Role

It is expected that the ICP Partnership Committee will meet at least quarterly and its role will be:

  • Set up and oversight of Multidisciplinary Working Groups (MDGs) to address the priorities of the ICP in line with Policy Direction
  • Oversight of risk Stratification of a defined population of service users
  • Agreeing processes for anonymised individual casework; care planning, case review and information sharing
  • Oversight of data collection and review in line with agreed performance metrics
  • Governance via the application of a Memorandum of Understanding (MoU)
  • Approve monitoring reports to LCG and Regional ICP Project Team
  • Approve service improvements brought forward by multi-disciplinary working groups for consideration by Local Commissioning Groups
  • Co-ordination of commissioner approved care pathways to ensure effective delivery and service improvement
  • Promote consistency of approach across MDGs

Membership

The membership of the Partnership Committee will initially be 12 members and the composition is designed to reflect the contribution which can be made by HSC Trusts and primary care independent contractors in relation to the key ICP objectives of Risk Stratification, Information Sharing, Care Planning and Evaluation (RICE). The involvement of voluntary, community and user representatives will bring the further opportunity to embed the aspirations and needs of communities and users in service provision.

The members of the ICP Partnership Committee will be supported in their work by the Business and Clinical Support Team

Membership of each ICP Partnership Committee will be

  • HSC Trust nominees :
  • 1 Medical specialist
  • 1 Nurse
  • 1 Allied Health Professional
  • 1 Social worker
  • 1 NI Ambulance Service
  • Primary Care :
  • 1 General Medical Practitioner (GP) appointed by HSCB following an expressions of interest process within the LCG area
  • 1 General Medical Practitioner (GP) elected by his/her peers. A Local Medical Committee (LMC) representative within the LCG area will be sought for this position.
  • 1 Community Pharmacist appointed by HSCB following an expressions of interest process within the LCG area
  • 1 Community Pharmacist elected by his/ her peers. A nominee within the LCG area will be sought from Community Pharmacy NI (CPNI) for this position.
  • 1 Voluntary and Community sector representative. A nominee will be sought from organisations within the LCG area by HSCB
  • 2 Users/carers representatives. Nominees from within the LCG area will be sought by HSCB.

To maintain appropriate Commissioner/ Provider separation ICP Partnership Committee members should not be members of the Local Commissioning Group.

A quorum shall comprise 7 members (If a member fails to attend 2/3rds of meetings without just cause, their membership will be reviewed and considered by the Chair.)

Period of Office

The term of office of members of the Partnership Committee shall be 3 years or such other period as may be determined by HSCB. Issues pertaining to resignation, cessation, removal or disqualification should be directed in the first instance by the Committee to the HSCB Business and Clinical Support Team.

Chair

The chair of the Partnership Committee will be elected by the committee members. The period of office will be 3 years subject to review.

Support

A Business and Clinical Support team employed by HSCB will support the work of ICPs and their FREDS Multi- Disciplinary Groups.
