2016 Hazelett Award for

Leadership in Grantmaking

Background: The Hazelett Award for Leadership in Grantmaking was created in honor and memory of Susie Hazelett, a leader in Indiana’s philanthropic community for many years before her death from cancer in 2004. The award was established to recognize the efforts of an individual who displays character and leadership in philanthropy, including:

§  A giving spirit displayed by mentoring and nurturing professional development in others

§  A giver of time, talent and other resources

§  A strong professional with the highest of standards, integrity and creativity

Nomination Process/Guidelines: Only grantmaking members of Indiana Philanthropy Alliance can nominate staff, board or volunteers of a member organization. Current Alliance Board members are ineligible for nomination. Attach to the nomination form no more than two pages of narrative (packet may also include no more than two supporting documents such as letters of support, résumé, etc.) that describe:

§  Commitment: The nominee’s length of service in the field

§  Mentoring leadership: The nominee’s demonstrated leadership in mentoring and nurturing professional development in others

§  Impact: Examples of how the nominee has given of his/her time, talent and other resources

§  High standards/integrity/creativity: Examples of how the nominee has demonstrated these characteristics

Selection and Recognition Process: A volunteer five-member review panel of grantmakers representing geographic and membership diversity (including, whenever possible, the previous Hazelett Award recipient) will review all nominations and select the award winner. The Secretary of the IPA Board of Directors will serve as the Award Selection Committee Chair; the other members of the Selection Committee will be approved by the IPA Board of Directors. The 2016 recipient will be honored at the IPA/GIFT Statewide Conference on June 7/8, 2016.

References should have a comprehensive knowledge of the nominee’s background and accomplishments. References for award finalists may be contacted at the discretion of the Award Selection Committee.
