COORDINATED PROGRAM REVIEW
CORRECTIVE ACTION PLAN
Criterion & Topic:
SE 24 Notice to parent regarding proposal or refusal to initiate or change the identification, evaluation, or educational placement of the child or the provision of FAPE / CPR Rating:
Partially Implemented
Department CPR Findings:
A review of documentation and interviews with staff members demonstrated that the district requires a parent to put in writing a request for a special education evaluation. Specifically, staff interviews indicated that once the written request is received, parents are then required to meet with staff before the consent to evaluate form is provided. This results in most cases with the notice not being sent within 5 days of the receipt of the referral. It was also indicated that in some instances parents are told that the student must go through the building-based instructional support system prior to having a special education evaluation offered.
Description of Corrective Action:
Training will be provided to all Principals and Special Education Department Heads. The 5-day response notice to evaluate will be monitored by Department Heads throughout the rest of the school year. In Fall 2013, referrals will be monitored monthly to ensure that this requirement is met.
Title/Role(s) of responsible Persons:
Director of Student Services / Expected Date of Completion:
01/30/2123
Evidence of Completion of the Corrective Action:
Records will indicate that timelines have been met.
Description of Internal Monitoring Procedures:

CORRECTIVE ACTION PLAN APPROVAL SECTION
Criterion:
SE 24 Notice to parent regarding proposal or refusal to initiate or change the identification, evaluation, or educational placement of the child or the provision of FAPE / Corrective Action Plan Status: Partially Approved
Status Date:11/01/2012
Basis for Partial Approval or Disapproval:
The district will provide training to Principals and Special Education Department Heads on sending the consent form with the proposed notice within five days of a referral for special education evaluations. The district cannot require the referral to be in writing and cannot delay the special education evaluation until the student completes the building-based team process. The district proposed internal monitoring on a monthly basis.
Department Order of Corrective Action:
The district must train staff that a referral for special education evaluation may be written or verbal. The procedures for staff to document the request for special education evaluations will be provided. The district may begin the building-based process at the same time as the special education evaluation, but cannot require that the building-based process be completed prior to beginning the special education evaluation. The district must send an evaluation consent form within five working days to a parent when the building-based support team is the source of the referral. Documentation from the building-based support team must be included in the student's special education file upon referral to special education.
Required Elements of Progress Report(s):
Provide training materials and the signed attendance sheet for the referral for a special education evaluation and provision of the consent form in five school working days from the oral or written referral for evaluation by January 29, 2013.
Submit the results of an administrative review of student records. Indicate the number of records reviewed, the number found to be compliant, an explanation of the root cause for any continued noncompliance and a description of additional corrective actions taken by the district to address any identified noncompliance. CORRECTIVE ACTION PLAN
Criterion & Topic:
SE 48 FAPE (Free, appropriate, public education): Equal opportunity to participate in educational, nonacademic, extracurricular and ancillary programs, as well as participation in regular education / CPR Rating:
Partially Implemented
Department CPR Findings:
Observations and staff interviews indicated that there is a small group of substantially separate students who are between the ages of 17 to 22 and their primary classroom is housed within the middle school facility. While these students do spend a portion of their week in a program located at a high school program within a neighboring district, the location of their in district classroom provides no opportunity for age appropriate integration with peers because they are located within a middle school level facility.
Description of Corrective Action:
The students ranging in age from 17 to 22 have been moved to a program at the Wellesley High School. This move occurred in June 2012. The new program is called Launch and is a Transition program that focuses on pre- and vocational skills, community experiences, including work and leisure activities. The program is staffed by a special educator with significant experience with this age group.
Title/Role(s) of responsible Persons:
Director of Student Services / Expected Date of Completion:
06/30/2012
Evidence of Completion of the Corrective Action:
This action has been completed.

Wellesley CPR Corrective Action Plan

COORDINATED PROGRAM REVIEW
CORRECTIVE ACTION PLAN
Criterion & Topic:
SE 55 Special education facilities and classrooms / CPR Rating:
Partially Implemented
Department CPR Findings:
Interviews with staff members and on-site observations demonstrated that at the Schofield and Fiske Elementary Schools there are instances where special education classes are not given the same priority as general education programs. Specifically, at the Schofield Elementary School, Room 122 is at times being used for provision of physical therapy services in one area of the room while other students are simultaneously receiving math and reading instruction. Observations at the Fiske Elementary School demonstrated that Room 36 has no ventilation and is located adjacent to the cafeteria with an excessive noise level at certain times of the day. Several of the students assigned to Room 36 have an IEP that identifies the need for a distraction free environment.
Description of Corrective Action:
For room 122 at Schofield, the Physical Therapy schedule will be reviewed to determine whether PT services are scheduled at the same time as academic instructional services. If the schedule interferes with instruction, a schedule change or room change will occur. Instruction at Fiske School will be moved from room 36 and relocated to another room in the building.
Title/Role(s) of responsible Persons:
Director of Student Services and Director of Curriculum and Instruction / Expected Date of Completion:
01/30/2013
Evidence of Completion of the Corrective Action:
Schedules for use of room 122 at Schofield will be submitted. The relocated space at Fiske School will be submitted.
Description of Internal Monitoring Procedures:
These corrective actions are finite and was accomplished will not require further monitoring.

Status Date:11/01/2012
Basis for Partial Approval or Disapproval:
The district will review scheduling for room 122 at Schofield to ensure that physical therapy can be provided in a separate space without other instruction. Students scheduled for room 36 at Fiske School will be moved to another room.
Department Order of Corrective Action:
Required Elements of Progress Report(s):
Provide floor plans for both schools indicating the location of services. If Schofield still provides physical therapy in room 122, provide the scheduling for that room by January 29, 2013.
The district will propose a date for an on-site review of facilities by the DESE no later than January 29, 2013.

COORDINATED PROGRAM REVIEW
CORRECTIVE ACTION PLAN
Criterion & Topic:
CR 6 Availability of in-school programs for pregnant students / CPR Rating:
Partially Implemented
Department CPR Findings:
A review of documentation showed that the district's policy requires pregnant students to obtain the certification of a physician that they are physically and emotionally able to continue in school and the district does not require such certification for all students for other physical or emotional conditions requiring the attention of a physician.
Description of Corrective Action:
The Wellesley Public Schools permit pregnant students to remain in regular classes and participate in extracurricular activities with non-pregnant students throughout their pregnancy. After giving birth, students are permitted to return to the same academic and extracurricular program as before the leave.
The district no longer requires a pregnant student to obtain the certification of a physician that they are physically and emotionally able to continue in school. The Wellesley School Committee revised Policy JGC in accordance with Massachusetts General Laws Chapter 71, Section 84.
Title/Role(s) of responsible Persons:
Salvatore Petralia, Assistant Superintendent / Expected Date of Completion:
05/22/2012
Evidence of Completion of the Corrective Action:
Pregnancies
Revised/Voted by School Committee 5/22/12.
"Pregnant students are permitted to attend classes and participate in extracurricular activities throughout their pregnancy. Exceptions will be made only when a physician expressly prohibits the student's participation."
"The District does not require a pregnant student to obtain a physician's certification that the student is able to continue in school. The District will provide reasonable accommodations for students with medical conditions relating to pregnancy to the extent such accommodations are provided to students with other temporary medical conditions. After giving birth, the student is permitted and encouraged to return to the same academic and extracurricular program as before the pregnancy leave."
The revised policy was disseminated to the school community through the district website on May 23, 2012.
