Directions for Testing

Dear Principal and/or Test Coordinator:

The Diocese of Allentown will use the Iowa Assessments™ (IA®) Form E, and Form F, and the Cognitive AbilitiesTest™ (CogAT®), Form 7, for spring 2016. Schools should have all materials necessary to administer the tests. If you need additional materials, please call Valerie Kosinski direct at 630.467.7079.

Iowa March 14 – March 18, 2016

Mailing Date: March 22, 2016

Required testing is as follows:

Grade 3 IA, Form F, Level 9 Complete Battery

CogAT, Form 7, Level 9

Grade 4 IA, Form E, Level 10 Complete Battery

CogAT, Form 7, Level 11

Grade 6 IA, Form E, Level 12 Complete Battery

Grade 7 IA, Form F, Level 13 Complete Battery

CogAT, Form 7, Level 13/14

Please note the following:

·  Students in grade 2 will write directly in the test booklet. There are no separate answer documents for them.

·  Students in grades 3–7 have received a Complete Battery Answer Document. All answer documents have a section for the Cognitive Abilities Test. However, the only students that will use this section of the answer document are those in grades 3, 5, and 7. Everyone else will leave the CogAT section blank.

·  Students in grade 3 will take the Word Analysis and Listening subtests. Directions to administer these tests are located on page 57 of the IA, Form E Directions for Administration, Levels 9–14.

General Information

To insure prompt scoring of test booklets and answer documents, it is imperative to record complete and accurate information.

Bar Code Labels

Students in grades 3,4,5,6 and 7 will use bar code labels. The name, birth date, and gender of each student are embedded in his or her bar code label. If you find an error in a bar code label, you may still use the label; however, you will need to provide a list of corrections to HMH/Riverside before the testing date. Please fax corrections to Valerie Kosinski at 630.467.7150.

Filling Out a Student’s Test Booklet/Answer Document for Grade 2 and New Students to the School:

·  Be certain the student’s name is spelled correctly, and the corresponding bubbles are darkened.

·  Be certain to darken the correct bubble for the gender of the student.

·  Be certain the correct birth date (Month, Day, and Year) is used. CogAT scores are age related. If there is an incorrect birth date or no birth date at all, the student will not receive age-related scores.

·  Use the Directions for Supplemental Coding sheet (pink) to code for special accommodations.

·  Be certain all subtests are complete.

Completing the Grade/Class Identification Sheets (blue)

·  Section 1, Information Box: Use March 9, 2016, for the Test Date. Print your school’s name for the Building Name. Use Diocese of Allentown for the System Name. Use the city where your school is located for the City.

·  Section 2, Class Name: Print the name of the teacher.

·  Section 3, Class Code: Leave blank.

·  Section 4, Grade: This should reflect the same grade as the students’ test booklets/answer documents.

·  Section 5, Form: Students in grades 2, 4, and 6 take Form E. Students in grades 3, 5, and 7 take Form F.

·  Section 6, Answer Document Count: Indicate the number of test booklets/answer documents you have under the grade/class identification sheet for this grade.

Building Identification Sheet (purple)

·  Use only one (1) Building Identification Sheet (purple). If you have more than one box for shipping, place this sheet on top of the documents in box #1. This identification sheet has been premarked for your convenience. Your school code number appears on this sheet. Please make a note of this number for your files.

Order Form for Scoring Services (green)

·  Use the special (green) Order Form for Scoring Services (OSS). A Dataset ID Number appears in the upper right corner of the OSS. This number has been assigned to your school and is used to track your answer documents and identify you as part of the Diocese of Allentown. It is imperative you use this OSS. This form, as well as the Building Identification Sheet (purple), must be placed in box #1. If you misplace the OSS, please call Valerie Kosinski immediately at 630.467.7079. Returning test booklets/answer documents without this OSS will result in scoring delays.

·  When collecting test booklets/answer documents, make sure you have one for each student. Check all the test booklets because sometimes documents are left in the test booklets and will not come to the scoring service.

Shipping Test Booklets/Answer Documents

·  Stack all test booklets/answer documents for each individual teacher by grade. There is no need to alphabetize the documents.

·  Place one (1) Grade/Class Identification Sheet (blue) on top of each teacher’s group of test materials.

Once you have organized your test booklets/answer documents, you are ready to package them in sturdy boxes. Make sure you pack the boxes securely to avoid shifting of documents in transit. Label each box as 1 of __, 2 of __, etc. (fill in the blank with the total number of boxes you are shipping). You may choose to package your answer documents in an envelope with your Grade/Class Identification Sheet on top to help keep the answer documents from tearing. It is recommended that you include all envelopes in a sturdy box along with your grade 2 test booklets. If you send individual envelopes to the scoring service you risk them being separated in transit.

Return your documents via UPS. Included in your packet are three (3) UPS/RS Labels. Place one label on each box you are shipping to the scoring service. Please refer to the UPS/RS Label Instruction sheet (goldenrod) for additional directions.
