The VSP Signature Plan offers eye examinations, eyeglassesor contact lenses and discountson other related services and products for a minimal copaymentor no additional cost.

Your Coverage and Co-Pays

Here’s what you can expect when visiting a VSP Member Doctor:

  • $10 co-pay for an annual exam(covered in full)
  • 100% coverage for one set of lenses per calendar year(single vision, lined bifocal and lined trifocal lenses)
  • $10 co-pay for frames (up to $150 every calendar year) plus 20% off any out-of-pocket costsOR$150 allowance for contact lenses per year (if you choose contacts lenses, you will be eligible for lenses and a frame one calendar year from the date the contacts were obtained)

VSP coverage also includes discounts on frames, sunglasses, laser vision correction contacts and hearing aids.

Member Doctors

Vision care services and vision care materials may be received from any licensed optometrist, ophthalmologist, or dispensing optician, whether Member Doctors or Nonmember Providers. You can choose any vision provider you wish. However, if you choose an eyedoctor who is not a member of VSP (a "Nonmember Provider"), you will pay more and may need to file your own claims. Identification Cards are Not Provided

Please note that VSP does not provide ID cards. To file a claim, just let yourdoctor know that you have VSP coverage. Your doctor will be able to file a claim using your social security number. You can also provide your doctor with your Windstream group number:30059925
