Chaplaincy Service - Pastoral, Spiritual and Religious Care
Last updated November 2016 / The Chaplaincy is fully committed to the health and well-being of all patients/service-users, their carers and staff under the care of the Trust, whether in hospital or in the community. Chaplains promote and support religious and spiritual care as vital aspects of health and well-being.

Continence Health Advisory Service

July 2016 / The Continence Health Advisory Service (CHAS) is made up of a team of specialist nurses who have skills in both assessment and treatment of bladder, bowel and pelvic floor problems.
An open access service, accepting referrals via electronic referral on SystmOne, or by letter, phone or fax.
Referral sources include GPs, practice nurses, consultants and any other health professionals.
First visit comprises a comprehensive assessment and a personalised treatment plan by one of our specialist nurses offering advice, support and the implementation of appropriate treatment and management, depending on the outcome of the assessment.

The Counselling Service aims to support individuals with a life limiting illness and / or families of individuals with a life limiting illness, through the provision of high quality counselling. The service is tailored to individual needs and aims to empower individuals and / or their carers by addressing their emotional, physical, psychological social and spiritual needs.

Dietetic Service

July 2016 / Our Dietetic Service comprises registered dieticians who provide specific dietary assessment and patient centred advice to promote nutritional wellbeing, treat disease and prevent nutrition-related problems.
We provide practical, safe dietary advice based on current scientific evidence, for the management of nutrition support, enteral feeding, textured modified diets, diabetes, weight management and a healthy well-balanced diet.

Doncaster Community Stroke Rehab Team

July 2016 / We provide high quality, individualised, interdisciplinary rehabilitation for Doncaster stroke survivors and their families in community settings.
Rehabilitation can be provided by our team for a maximum of six months following a diagnosis of stroke. After this time, we refer patientswith ongoing rehabilitation needs to the appropriate service according to the Doncaster agreed stroke pathway.
The amount of visits received will depend on individual needs identified on assessment, and therapy is carried out as agreed with the patient around their set personal goals.

Epilepsy Specialist Nurses
Last updated November 2016 / We are a team of qualified nurses who also have training and experience / expertise in the management of epilepsy. We work across the Doncaster borough with neurologists, GPs, the private, public and voluntary sectors, and most importantly with the patients and their families / carers.
Our epilepsy nurse specialists play a key role in supporting patients with epilepsy across primary, secondary and tertiary care in managing their condition. We support consultant neurology clinics, run nurse led clinics, work in the community and offer a telephone clinic/support service.

Falls Clinic Evergreen

Hepatitis C Clinic
Last updated November 2016 / The Hepatitis C Service plays a key role in supporting patients through treatment across primary and secondary care, as well as in local prisons.
The service provides:
· Nurse led clinics with hospital specialists (consultant).
· Appointment based community clinics
· Prison in- reach.
· Home visits.
· Advice, support and advocacy via telephone.
The aim of the service is to:
· Support patients with newly diagnosed Hepatitis C to understand their condition and what it means to them.
· To educate people about liver disease and how their lifestyle can impact on this, and how they can improve it.
· To enable all patients diagnosed with Hepatitis C to access treatment.
· To be a resource for educating all.

Intermediate Care Services
Last updated November 2016 / Our Intermediate Care Services comprise ourintermediate care rehabilitation wards: Hazel and Hawthorn.
Hawthorn and Hazel Wards provide a rehabilitation service to patients either from within the community or from an acute hospital. The wards provide a nurse/therapy led holistic pathway for inpatient rehabilitation to people who have an illness or who have fallen, and require rehabilitation from the multi-professional team to return home independently or with support from social services or reablement team.
