  1. Pre-Conference with Clinical Instructor at Medical surgical Unit Conference Room
  2. Obtaining patient assignments.
  3. Discussion of clinical objectives per syllabus.
  1. Participate in nursing rounds and receiving of client’s reports from outgoing noc nurse – endorsement. Ask/ or listen for the following:
Medical Dx, reason for hospitalization (cc), IV, needle gauge, site, what is the IV hanging, special procedure, diet, activity, precautions in place, is the client NOP or feeder, has GT or NGT? On O2 How much? Via what ? Latest lab values of concern, allergies? Code?
2. Do focused client assessment and take Vital
Signs.
3. Check with primary nurse if OK to give oral
medicationsand check oral meds available.
Review meds listed in the MAR and available
meds at the medication room.
History and Physical, MD orders, labs and
diagnostic procedures done for the client,
review MAR to ensure completeness of all
medication entered/orders . Check with
instructor/primary nurse any question with
medications.
  1. Provide with AM Care (bed bath, oral hygiene, hair hygiene). Check if there is any contraindication. Pay attention to client with ISOLATION PRECAUTIONS. Check with primary nurse before assisting client to get up or check orders pertaining to activity that they can do. Make sure to ask for any mobility restrictions. Continue with pt. assessments. Pain if any, etc.
  2. Continue with client AM care.
If client has pain or discomfort- assist in
repositioning, turning if not contraindicated.
Assess for need of pain meds and report to
primary nurse. Assess for pain location,
intensity, onset, quantity, description.
PASS MEDS oral only (when skills have been
checked off and performed in the skill lab. May
start to do giving IV meds that are not life
threatening e.g. anti nausea meds or flushing of
IVs and hanging a new IV bag).
  1. Do check for treatments necessary
  2. Prioritized clients going for surgery or special procedure to make sure meds are given, vital signs are taken, check orders to ensure completeness of care needed.
  1. ACCUCHECK. Checking Blood Capillary Glucose Level. Obtain code from the instructor. Make sure accu-check machine is calibrated before use for testing.
  2. Report to primary nurse obtained BSL result. Log onto Blood Sugar Level Flowsheetobtained result.
  3. When allowed to give meds, start administering insulin at 1145 ( Check type of insulin).
LUNCH BREAK
If unable due to some procedure, may follow. Endorse clients to primary nurse to ensure safety of clients.
  1. Start mock documentation ( review charting from previous day. Study and compare).
  2. Continue with patient care, assist in treatments needed.
  1. Gather data for your Care Plan/Study medications. Complete your nurses notes to be reported during post conference.
  2. Continue with patient care.
  3. Provide health teaching/instruction to clients if appropriate.(Check with instructor prior to doing so).
  4. Assist with taking Vital Signs
Post – Conference with Instructor at the Medical- Surgical Conference Room.Study about : CORE MEASURES, National Patient Safety Goals, Turn in quizzes from the pbworks site 120 and 121. Discuss CP and concept mapping, Medication and MED-MATH QUIZ/ Do Practice Nursing care Plan
Check for student evaluation form.
Verbalization of concerns and giving report of client care, etc.
End of clinical rotation for the day.

Note: Schedule may change according to needs of client, availability of clients, unexpected emergency situation, change of client condition, staff prerogatives, therefore flexibility is a must. Respect, collaboration, compassion, courtesy, safety and effective care are to be observed at all time. Proper clinical uniform is a must. Proper appearance will be checked.
