Locum Specialty Registrar in General Medicine - Locum Appointment for Service (LAS)

Equivalent to FY2 or ST1 level

05 August 2015 – 2 February 2016

THE POST

Ideally we require Locum Foundation Year 2 Doctor / Specialty Registrar in General Medicine, Locum Appointment for Service (LAS) from August 2015 for 6 months (or shorter duration). You would gain wide experience in General Medicine in a District General Hospital setting and also participate at the intermediate level (FY2 or SHO) in the Hospital at Night Team (HaN Team).

This post is aimed at FY2 or ST1 level i.e. those doctors having completed at least the first year of a Foundation medical training programme (or equivalent) and having being assessed as "acute take competent" under supervision. Candidates may be looking for some experience of general medicine in which they may have a longer term interest (e.g. future entry into core medical training), or to provide a professional development supplement to other career options longer term. The post may be most suitable for FY2 or ST1 level doctors seeking out of programme experience, to support future career intentions.

You will be one of the 5 members of the core HaN team (operational from 9.00 pm – 9.00 am), the others being the Team Leader (Registrar in Medicine/DME or a senior GPST), 1 x FY1 and 2 advanced nurse practitioners (one of whom coordinates the team). Whilst the majority of overnight activity happens in medicine (about 75% of overnight activity) the HaN team provide a generic clinical service to all adult inpatient areas in BGH (excluding ITU) with specialty support provided from staff grades/specialty trainees in general surgery, O&G, paediatrics, orthopaedics and anaesthetics. An orthopaedic trainee covers A&E with support from a salaried Emergency Medicine GP.

Whilst this post has no educational and Dean’s approval, there is an excellent Postgraduate educational programme, good library facilities on the Borders General Hospital site and protected formal teaching in the Department.

This is a full shift working pattern with a Band 50% supplement and a 1 in 10 commitment to night duty and 1 in 8 commitment to weekends.

BORDERS GENERAL HOSPITAL

330 bed acute district general hospital situated centrally in the Borders region at Melrose, which is 37 miles south of Edinburgh and the nearest Medical School.

The hospital provides facilities for the following specialties:- General Medicine, Palliative Medicine, Medicine for the Elderly, (it also contains a General Practice ward), General Surgery, Orthopaedic Surgery, ENT Surgery, Obstetrics and Gynaecology, Paediatrics, and Ophthalmology and a Day Procedure Unit. A major forthcoming development is the building of new integrated clinical area for Accident & Emergency, Acute Receiving Unit and Primary Care Emergency Service – known as the Borders Emergency Care Centre. NHS Borders has recently appointed its first Consultant in Emergency Medicine.

Each Unit carries the approved Consultant and Junior staff cover with the supporting diagnostic treatment and other services of Radiology, Laboratory, Rehabilitation and Pharmacy etc. There is an Intensive Therapy Unit and provision for Renal Dialysis.

Other specialist services for the Borders Area are provided from Edinburgh on a part-time visiting specialist basis by arrangement with NHS Lothian.

DEPARTMENT OF MEDICINE

The Medical Unit has 110 beds of which 12 are in a high dependency area and 38 are the in the Medical Assessment Receiving Unit. In addition, there are 8 beds in an Intensive Care Unit shared with other specialties. There are 30 beds in Medicine for the Elderly, 12 Stroke beds and 8 palliative care beds.

CLINICAL DUTIES OF POST

·  Clinical responsibility for the care of patients in the medical unit. This includes daily ward round, admissions of list cases and emergency admissions (according to rota).

·  On basis of rota, clinical responsibility for medical emergencies in the Accident and Emergency Department and initial assessment and appropriate referrals when necessary of medical cases.

·  Work in out-patients under Consultant supervision in General Medicine and Specialist Clinics, namely diabetes, cardiology, respiratory medicine and gastroenterology.

·  Participation at intermediate level (FY2 / SHO) on the Hospital at Night Team rota.

·  Organisational responsibilities, e.g. PACES examinations, rota preparation and FY1 / FY2 (if applicable) supervision.

STUDY LEAVE & PROFESSIONAL DEVELOPMENT - SPECIALTY REGISTRARS (LAS)

Study leave is a maximum of 5 days in the period of this appointment. It can be used for the purpose of studying and/or sitting an examination for a higher qualification.

There is a well equipped library in Napier University within the hospital grounds available for study and reference purposes. The Board's Librarian is also available if required.

There is a weekly Clinical Meeting. Each consultant has a formal weekly Teaching Ward Round. Each month there is an PG educational half day when clinics and other sessions are cancelled to allow ALL to attend except when involved with emergencies.

This post is not recognised for training purposes but there is an excellent Postgraduate educational programme, good library facilities on site and protected formal teaching within medical unit. A personal development plan will be agreed with the successful and is flexible according to the needs and interests of the successful applicant. For example may take the form of support with membership examinations (through study leave), study leave for shadowing other specialties in the NHS Borders / wider South East area. An educational supervisor (a senior clinician within the medical unit) will be appointed.

If desired we would support the postholder in a training opportunity, the TACTICS course held at Larbert, which is being rolled out to CMT trainees in the West of Scotland to support them undertake the medical register role. The link is below –

The skills are not just about dealing with medical emergencies but about prioritising tasks e.g. dealing with relatives etc –the feedback for the course from the trainees has been very positive. The successful candidate can book on-line and NHS Borders will reimburse the course fee and expenses if you book on to the course either before or after your LAS post at BGH starts.


CONDITIONS OF SERVICE

Current salary scale for Specialty Registrar: Current salary scale for Specialty Registrar (higher level) LAS: £1,238.75 per week, inclusive of banding 50% supplement (equates to £64,591 per annum).

Contracted hours of duty: Basic working week and an out of hours commitment on nights full shift and MAU/Ward weekends, which includes prospective cover.

Duration: The appointment is for 6 months but applications for lesser duration also accepted.

Residence: The post is not compulsory – single accommodation will be provided at reduced rental charge in the Hospital residencies if this is suitable. Financial assistance with relocation if moving to the Borders area is available. Alternatively support with travel expenses may be applicable.

Superannuation The post is superannuable unless the postholder opts out of the scheme or is ineligible to join and remuneration will be subject to deduction of Superannuation contributions in accordance with the NHS (Superannuation) (Scotland) Regulations.

Leave Entitlement There would be 35 combined annual leave / public holidays in the duration of this appointment if 12 months.

General Medical Council All medical and dental staff must be registered with the General Medical Council with a licence to practice. This certificate must be produced at interview, when you commence work and on subsequent occasions as requested. This also applies to proof of re-registration of the annual subscription.

Occupational Health and Exposure Prone Procedures (EPP): All entrants to NHS Borders must be certified medically fit, and the appointment is conditional on such certification; therefore the applicant will require to be assessed by an Occupational Health Service (normally by questionnaire but occasionally an appointment will be required.) The holder of this post may be required to undertake on a regular basis or occasional basis exposure prone procedures (EPPs). As this could potentially place patients at risk, candidates must show evidence of appropriate immune status, or agree to undergo the necessary procedures. This is checked by the Occupational Health Service.

Medical Indemnity: You are normally covered by the NHS Hospital and Community Health Services indemnity against claims of medical negligence. Health Departments advise that employees maintain membership of a defence organisation, as in certain circumstances (especially in services for which a fee is paid) the doctor may not be covered by the indemnity).
