Essay: This scholarship will be awarded to a student who is well-rounded and exemplifies the PTA spirit of community service and volunteerism. Attach a 1 to 2 page essay describing your high school activities and commitments to LMHS and/or the surrounding community. Tell us how this participation has improved your school, community, and/or yourself.

Personal References: Obtain two letters of reference, one from a high school staff member or guidance counselor and one from a community member (non-relative) such a clergy, employer, neighbor, etc.

Transcript: Include a copy of your LMHS transcript.

Value of Scholarship

The LMHS PTSA Scholarship is a one year non-renewable scholarship for a total of $1,000, awarded to one student.

Requirements

To be eligible, an applicant must:

·  Be a resident of Florida

·  Be a United States Citizen

·  Be a student member of the LMHS PTSA as of February 15, 2016

·  Be a graduating senior of LMHS

·  Have a minimum unweighted GPA of 2.5

·  Submit an application, essay, 2 letters of reference, and transcript to be received no later than end of school day on March 31, 2016.

Steps to Apply

·  Type or print application. Be sure to answer all questions.

·  Enclose in a large envelope the following:

a.  Application

b.  Essay

c.  Two letters of reference – one from school, one from community

d.  LMHS Transcript

·  Mail or turn in completed scholarship packet no later than March 31, 2016 to:

Notification

The scholarship will be awarded during the 2016 Awards Night ceremony at LMHS. No other acknowledgements will be made.
