The section utilized by Physicists and Dosimetrists will be the Clinical part of ONCOCHART®. In there, the Calendar will be employed which is an intuitive and manageable tool to select narratives and record procedures, over which they have responsibility. Procedures are scheduled and billed, plans implemented and reports generated. To get to the Calendar, select the Patient first.

Patient Navigator

1.  Click on the Patients Icon.

2.  Double click on selected Patient. (It will bring up the Patient Navigator)

1.  Click on Clinical then Calendar.


Isodose Plan 77305, 77310, 77315

1.  From the Calendar, click Isodose Plan.

2.  Select correct type (Conventional Isodose, Brachy, etc.).

3.  Step through the tabs; answer the questions that apply to that patient.

4.  Your answers will determine the complexity of the plan and select the correct CPT Code for Billing.

3-D Simulation (Dosimetry) 77295

2.  From the Calendar, click Simulation.

3.  Click & Drag 3-D to the Calendar.

4.  Answer the questions on the tabs that relate to that patient’s case.

Note: If you answer the question on the Summary tab, a short generic Narrative will be created and you do not need to answer any other questions. Remember, the more questions you answer the better the quality of your documentation.

Basic Dosimetry 77300

1.  If you answered the Dosimetry or Summary questions under Treatment Devices, it is not necessary to do a separate Basic Dosimetry Narrative, which would send another 77300 to Billing.

2.  If you have not answered the Dosimetry questions under Treatment Devices, or need to bill additional Dosimetry charges, proceed as follows:

a.  From the Calendar, click Basic Dosimetry (MU Efficiency Index) to a date on the Calendar.

b.  Drag & Drop Basic Dosimetry to a date on the Calendar.

c.  Select Billing or Narrative.

d.  Type in the number of Dosimetries.

3.  The Narrative form will appear with the number of tabs corresponding to the number typed for number of Dosimetries. Answer the questions.

a.  Click Save and Send to Billing – this sends the 77300 CPT Code to the Billing Log, for each Dosimetry Recorded.

IMRT Dosimetry (77301)

5.  The Physician must complete the Clinical Treatment Plan for IMRT or Tomotherapy and the plan must include at least one Critical Dose Point for this button to be active.

6.  Click & Drag IMRT Dosimetry to a date on the Calendar.

7.  Select Billing or Narrative.

8.  It is strongly recommended that you select Narrative.

9.  Select the area of treatment (Head & Neck, Prostate, etc.).

10. Step through the tabs; answer the questions that apply to that patient.

11. Click Save and Send to Billing. This will send the CPT Code 77301 to Billing.

Diodes (Micro Dosimetry) 77331

1.  From the Calendar, Drag & Drop MicroDosimetry to a date on the Calendar.

2.  Type in the number of MicroDosimetry procedures performed.

3.  Send to Billing or create a Narrative.

4.  Step through the tabs; answer the questions that apply to that patient.

Special Physics Report 77370 – may occur at any point during Treatment at Physician’s request

1.  From the Calendar, click Special Physics Consult.

2.  Drag & Drop Special Physics type (3-D CRT, Brachytherapy, Custom, IGRT, IMRT, etc.) to a date on the Calendar.

3.  Step through the tabs; answer the questions that apply to that patient.

4.  Technical charge is sent to Billing.

5.  Generally only one allowed per course of therapy.

Physics Support (Continuing Physics) 77336

1.  From the Calendar, click Continuing Physics.

2.  Drag & Drop Continuing Physics to a date on the Calendar.

3.  Step through the tabs; answer the questions that apply to that patient.

4.  Technical charge is sent to Billing.

5.  Generally only one allowed per course of therapy.
