Step 1.The participant meets the host provider
The participant considers the level of responsibility they are comfortable with, chooses and contactsa host provider that best suits their needs.
The host provider requeststo see the letter of informationfrom the participant provided to them by Disability Services.
If the participant does not have their letter, the host providercan contact Disability Services to confirm the information provided.
The host provider and theparticipant should discuss:
  • if the host provider offers the services the participant is seeking
  • the supports the host provider offers and the costs of the supports.

Step 2. The participant chooses their host provider and Disability Services is advised
The chosen host provider or the participant will need to let Disability Services know, in writing, the fees and charges for 2.066 (administration), 2.067 (any initial establishment fee) and the intended start date via a letter or email.

Step 3.The participant, the host provider and Disability Services prepare to commence the arrangement
Two processes need to be completed for the participant to start self-directing their supports through YLYC.
  1. The administrative process to set up a person’s funding:
This is completed by Disability Services and may take up to six weeks. Host providers should consider this time frame when discussing a start date with the participant.
  1. Developing an agreement:
The host provider and the participant develop and sign the agreement together.The participant and the host provider agree on the terms of the agreement which includes each party’s responsibilities and particular support being provided.
A host provider will need to ensure this agreement provides sufficient information and safeguards* to meet all their responsibilities such as requirements of the Disability Services Act 2006, the Community Services Act 2007, the Human Services Quality Framework and the host provider organisation’s Service Agreement with the department.

Step 4.The host provider organisation signs the output schedule
Disability Services will send the host provider an output schedule summarising the value of funding allocated to the participant for outputs 2.066, 2.067 and 7.04. Allocating a 7.04 enables planning to occur with the participant.

Step 5.Payment is made to the host provider organisation as YLYC subtype grant
Disability Servicesmakes the funding paymenttothehost provider organisation to their host provider outlet.
Step 6. The host provider and the participant develop a plan
The host provider works with the participant to develop a plan to meet the participant’s assessed needs.
The plan forms part of the agreement between the host provider and the participant and identifies the agreed purchases.
Planned purchases must meet the:
  • participant’s disability services assessed need
  • requirements of the Disability Services Act 2006,including having appropriate safeguards in place
  • requirements of the restrictive practice policy, where a restrictive practice is in place
  • requirements of the organisation’s Service Agreement, including theYLYCHost Provider Handbook.

Step 7. The host provider completes and submits a Service Type Form
Within 28 days, the host provider is required to complete the service type form detailing the services to be purchased and theirestimated quantities. This form is used to amend the grant to the host provider.
The host provider emails the service type form to the regional YLYC mailbox relevant to the participant’s location.(Regional YLYCemail addresses are listed on the service type form template). / See Service Type form template

Step 8.The host provider organisation submitsthe quarterly reporting requirements
The organisation provides a Director’s Certificationtothe department. The submission of the Director’s Certificate supports quarterly payments. Non-compliancemay affectthe payment for the next quarter.
OneDirector’s Certification is requiredfor theorganisation (providergroup). Aseparatecertification is notrequired foreachprovider outlet. By signing the Director’s Certificate, the organisation certifies they have sighted the financial and performance data for the period and that the information sighted is a true account of the organisation's financial position and performance as it relates to departmental funding. It also certifies the funds have been used as agreed in the Service Agreement.

Step 9.The host provider and the participant review the plan
Together, thehostprovider andtheparticipantreview the participant’splan. This could include reviewing:
  • support quality and quantity
  • change of circumstances
  • the provision of care and support where a restrictive practice applies to ensure requirements of the restrictive practice policy are met
  • budget and financial planning.

Where a review identifies the need to change a participant’s service types, a new service type form is required.Changes to service quantities (outputs)do not require a new form as a person is able to buy more or less of a particular service type already on their form. / For information about what to do when a harmful behaviour is identified, refer to: the Your Life Your Choice – Use of Restrictive Practices Policy (Appendix 1 - YLYC Host Provider Handbook)
