The Bureau of Fire Protection Regional Office-10, through its Bids and Awards Committee,invites contractors/bidders for theRepair and Maintenance of Yumbing Fire Sub-Station, Yumbing, Mambajao Camiguin.

Please submit your quotations/proposals duly signed by you or your duly authorized representative not later than 10 October 2016, at exactly 9:00 AM for the scope of works described below, subject to the Terms and Conditions provided at the dorsal portion of this request for quotation (RFQ).

A copy of your Mayor’s/business permit, PhilGEPS Registration Number, Income/Business Tax Return, PCAB License and Omnibus Sworn Statementis also required to be submitted along with your quotation/proposal manually or through facsimile or email at the address and contact number indicated below.

Terms and Condition

1. Bidders shall provide correct and accurate information in this form.

2. Price quotation/s must be valid for a period of thirty (30) calendar days from the date of submission.

3. Price quotation/s to be denominated in Philippine Peso shall include all taxes, duties, and/or levies payable.

4. Quotations exceeding the Approved Budget for the Contract shall be rejected.

5. Award of contract shall be made to the lowest quotation which complies with the scope of works and other terms and conditions stated herein.

6. Upon opening of bids the contractor will submit the Affidavit of Site Inspection.

7. The winning contractor is hereby required to provide within ten (10) calendar days upon receipt of the Notice of Award (NOA)the Performance Security in the form and the amount stipulated in Section 39.2 of the IRR of RA 9184.

8. The advance payment/mobilization of the contractor in an amount not exceeding fifteen (15%) of the total contract price will be release upon a written request of the contractor which shall be submitted as a contract document.

9. Progress payments are subject to retention of ten percent (10%) referred to as the “retention money” shall due for release upon final acceptance of the works.

10. The contract duration required ninety(90) calendar days, to be reckoned from the date of receipt of the Notice to Proceed.

11. Current year Mayor’s/business permit, PhilGEPS Registration Number, income/Business Tax Returns, PCAB License and Omnibus Sworn Statement shall be attached upon submission of the quotation.

12. Mode of Payment

a. 15% Mobilization

b. 50% Progress payment - upon submission of Statement of Work Accomplished (SWA)

c. Final Billing

13. The BFP shall have the right to inspect the repaired fire stationto confirm their conformity to the technical specifications & satisfies the scope of work to be done.

14. Liquidated damages equivalent to one tenth of one percent (0.001%) of the contract price shall be imposed per day of delay. The BFP shall rescind the contract one the cumulative amount of liquidated damages reaches ten percent (10%) of the amount of the contract, without prejudice to other courses of action and remedies open to it.
