Target Audience:

This educational activity is designed for nurses, HF program coordinators, cardiologists, program medical directors, quality improvement personnel. Any allied health professional with an interest in improving care related to the topic area is welcome to attend.

CNE Outcome:

Participants will be able to evaluate the benefit of providing targeted assistance to Congestive Heart Failure (CHF) patients in the home setting after discharge to reduce readmissions. This outcome will be assessed by participant self-report of knowledge gained from the activity.

Objectives - following this educational series, participants will be better able to:

  1. Discuss the importance of engaging heart failure patients in their care
  2. Describe techniques to actively engage heart failure patients in the medical care plan
  3. Communicate with a patient/family about how they are able to manage heart failure symptoms as a result of actively participating in their care
