This is the Equality and Diversity Monitoring Form for the Education and Training Foundation. This is the form that we will use to collect data on equality and diversity within the Foundation.

In accordance with our Equal Opportunity Policy, we undertake monitoring to ensure that discrimination does not occur because of race, sex, sexual orientation, gender reassignment, religion of belief, marital or civil partnership status, age, disability or pregnancy and maternity.

We would be grateful, therefore, if you could complete the questions on this monitoring form. We recognise that you may feel that the information requested is sensitive. Completion of the questionnaire below is entirely voluntary. Neither the act of collecting this information nor any of the questions is intended to cause offence. An option to ‘prefer not to answer’ is included for every question.
