Position Objective
The Reconnect Case Manager will work within the Box Hill Institute (BHI) Reconnect service tosupport young people to identify and address barriers to successful participation in education, employment and training.
The key purpose of the positionis to provide specialist assessment, intensive case management and outreach to early school leavers engaged in the Reconnect Program.
Role Responsibilities:
Key responsibilities for this position include:
- Marketing the Reconnect program through networking and direct outreach with the target group.
- Providing case managmentsupport to Reconnect students through assessment, individual support plans, counselling, outreach and supported referral.
- Providing support, information and advice for carers and families.
- Meeting with each young person once per month to review and update the support plan.
- Delivering Reconnect in-class health and wellbeing education modules.
- Case notes, file maintenance and data collection.
- Attending staff meetings, case planning, seminars and professional development workshops.
Key Selection Criteria
- Relevant Health and or Welfare tertiary qualification e.g. in Social Work, Psychology orCounselling.
- Extensive experience in youth services including working with vulnerable young people and those from diverse backgrounds including ATSI, CALD and LGBTI communities.
- Experience in providing outreach support to young people.
- Knowledge of available interventions and support services appropriate to young people.
- High-level verbal and written communication skills, including the ability to present to groups and work with clients from diverse backgrounds.
- Proven administrative skills, including reporting and data collection.
- A desire to be innovative, learn and develop new skills.
Essential Requirements
- Satisfactory outcome of a confidential Police Check and Working with Children Check.
- Certificate IV Alcohol & Other Drugs, or willingness to complete.
- Empathy for those whose lives have been affected by problematic alcohol and other drug use and/or mental health disorders.
- Possession of a current Victorian Driver’s License.
- Information technology skills, including proficiency in Microsoft Office suite.
- Eligibility to work in Australia.
Desirable Requirements
- Specialist knowledge in one or more relevant areas such as drug and alcohol, mental health, family violence interventions, attachment and trauma-informed practice, child and family developmental processes, understanding risk and protective factors for young people.
- First Aid Certificate.
- Current knowledge of OH&S practices.
