2009 YWCA Outstanding Achievement Award Nomination Form

The annual YWCA Outstanding Achievement Awards are given to remarkable women in the greater Salt Lake area whose lives and achievements exemplify the YWCA’s mission: eliminating racism, empowering women, and promoting peace, justice, freedom, and dignity for all.

To nominate an outstanding woman, please complete the following information, attach a resumé or vitae and at least two letters of recommendation, then send to the YWCA. The deadline is 5:00 p.m. Monday, June 1, 2009. Please do not send additional materials. Award winners will be selected by a committee whose members include 2008 OAA award recipients, one YWCA Board member, and one YWCA staff member. Nominees will be judged on five criteria: 1) exemplifying the YWCA mission; 2) demonstrated support of other women; 3) professional achievement; 4) peer recognition; and 5) personal growth. To receive this award, an OAA award winner must be able to attend the YWCA’s annual LeaderLuncheon on Friday, September 18, 2009. If nominating a woman for the Racial Justice Award, please also explain how her life has demonstrated a commitment to racial justice, and how she has set a personal example of courage, integrity and steadfastness in supporting positive social change to end racism.

If nominating a young woman (18-30) for the Young Women’s Leadership Award, please also describe how she has demonstrated her desire to effect change and improve the lives of others, and how she has become a leader or role model for other young women.
