Submarine Safety Survey Information

All SSN and SSBN submarines receive a safety survey every 24 months.

Pre-Commissioning units should complete their first safety survey before INSURV or completion of PSA.

Request access to ship, shipyard, and submarine piers. Security clearances for COMNAVSAFECEN are submitted via JPAS. Survey team will require access to engineering, radio, ESM and weapons spaces.

General survey schedule:

0745 - Survey team arrival and review of safety documents and instructions

0800 - In-brief for CO, XO, Department Heads, Safety Officer, COB (others as designated by CO)

0815 - Survey team members accompanied by their counterparts (E-6 and above) conduct

survey, conduct PMS monitors, discuss program operations, distribute handouts and

accomplish 1 on 1 training

1000 - Senior Surveyor accompanied by Safety Officer and/or Executive Officer conduct Safety

walk through

1100 - Working lunch (coordinated with ship's schedule)

1200 - Supply Officer - HAZMAT survey of the ship

1200 - Continue survey walk-through as required

1400 - Survey team members enter results

1500 - Survey team meeting with Senior Surveyor

1630 - Out-brief for CO, XO, Department Heads, Safety Officer, cob (others as designated by CO)

Department heads, Division Officers and Chiefs are highly encouraged to accompany during surveys.

The following key players should plan on participating in the survey for six hours: Safety Officer, Ship's DCPO, and DCA.

The following supplemental players should be available to support the safety survey for approximately two hours: 3MC, MDR, HAZMAT Coordinator, A-division LPO, M-division LPO, E-division LPO, ET-division LPO, Deck LPO / 1st LT, and TM-division LPO.

To receive full credit for both blue and gold crews, personnel, listed above, from both crews are required to be present on the day of survey.

Each submarine shall provide a copy of the survey results to your ISIC within 30 days, with the written status of all discrepancies identified during the survey. Request copy is sent to Naval Safety Center and TYCOM (Attention: Mr. Tom Deatherage, N451)

In this era of constrained resources, and to maximize cost effectiveness, request ISICs coordinate multiple surveys for our survey teams to travel to a geographic area. In those submarine home ports that have multiple submarine squadrons, request ISIC and TYCOM staffs coordinate safety survey schedules to maximize the number of ships visited while in that area.

To schedule a safety survey, submit a naval message to COMNAVSAFECEN two months in advance of requested dates. This will permit coordination of scheduling, funding, and travel. Early coordination is encouraged with your area POC:

Units are highly encouraged to request a safety survey or assist visit within two weeks of exiting dry dock, following an extended availability, prior to major inspections, as follow-up to a safety survey, or whenever your command desires our support. Assist visits are very supportive of a new CO, XO, or COB. They provide a full report on the ship's safety programs, material status, and training with very little disruption to the crew. Additionally, it accomplishes the command's annual safety self-assessment requirement. Our submarine survey team will work with and train your crew to complete the task in six hours.

Include the following information in message:

a. Dates available (primary and secondary)

b. Location of survey (e.g., pier side, shipyard, or other abnormal location)

c. Operational status (e.g., upkeep, dry dock, post overhaul)

d. Ship's safety officer, 3MC and POC for security badge to include

(DSN and commercial, e-mail address (.mil) prefer unclassified, and fax numbers)

e. Focused area of concern

(i.e., electrical, deck, motorcycle safety, HAZMAT, and damage control readiness)

f. Conflicting events scheduled during the survey

(i.e., weapon handling evolution, SMI, or nuclear testing)

g. Requested training

(additional time required outside survey for PMS is 2 hours)
