SUBJECT:Revised Custodial Parent Notice of Intent to Discontinue IV-D Services (Form CS-137)

Revised Direct Deposit Activation Notice (Form CS-167) and Procedural Instructions

The CS-137, CS-167 and their procedural instructions have been revised to update the format and to reflect technical changes to be consistent with other forms and procedures. The CS-137, CS-167 and their procedural instructions have been updated on the Division of Child Support’s Intranet Web page. The Spanish versions of each form will be added to the Intranet once they have been translated by the Cabinet for Health and Family Services Language Access Section.

The forms and their procedural instructions were sent out for review and comments considered and included in the revised forms.

The following revisions have also been made to the CS-137, CS-167 and their procedural instructions:

• The third paragraph of the CS-137 was reworded for clarity and to add that non-MA Only clients are not required to complete an application if services are requested again at a later date.

• The procedural instructions for the CS-137 have been revised to change “GOT” KASES Help Desk to “CHFS” KASES Help Desk, and the reference to KASES Handbook was changed to KASES Systems Guide.
• The procedural instructions for the CS-137 were revised to add that the CS-137 is automatically generated for MA Only cases as well as K-TAP cases.
• The “Unbridled Spirit” logo was added per CHFS Secretary’s instructions.

This transmittal letter is to be saved and filed for future reference as it explains changes that have been made to the Kentucky Child Support Forms.

The Kentucky Child Support Forms are to be updated as shown below and on page two.
