PROCEDURE 3-14:Collecting Blood for Culture

Equipment/Supplies: Specimen container as required by the laboratory and test ordered (yellow-top sodium polyanetholsulfonateVacutainer tubes or aerobic and anaerobic blood culture bottles), blood culture skin prep packs (or 70% isopropyl alcohol wipes and povidone–iodine solution swabs or towelettes), venipuncture supplies (see Chapter 41), gloves, biohazard sharps container; biohazard waste container

Standards: Given the needed equipment and a place to work, the student will perform this skill with _____ % accuracy in a total of _____ minutes. (Your instructor will tell you what the percentage and time limits will be before you begin.)

1. Check the physician order. Assemble the equipment and supplies. /  /  / 
2. Wash your hands. Put on gloves. /  /  / 
3. Greet and identify the patient; explain the procedure; answer any questions /  /  / 
4. Verify that the patient had not initiated antibiotic therapy; if therapy was started, document the antibiotic, strength, dose, and duration and time of last dose /  /  / 
5. Using skin preparation kits or supplies, apply alcohol to the venipuncture site and allow it to air dry; applypovidone–iodine prep in progressively increasing concentric circles without wiping back over skin that is already prepped; let it stand at least 1 minute and allow it to air dry without touching the skin after preparation /  /  / 
6. Wipe the bottle stoppers with povidone–iodine solution /  /  / 
7. Performvenipuncture /  /  / 
8. Fill the bottles or tube according to specific laboratory procedure; invert each one eight to 10 times as soon as collected; if using culture bottles, fill the aerobic bottle first /  /  / 
9. Complete the venipuncture; use an isopropyl alcohol wipe to remove residual povidone–iodine the from skin; labele the specimen with the patient's name, date and time of collection, the origin of specimen /  /  / 
10. Repeat steps 5 to 9 at a second venipuncture site within 30 minutes of the first collection /  /  / 
