PAYMENT DETAILS

All monies must be received before the placement of your child in the crèche. If you cancel with less than 24hrs notice you are required to pay a cancellation fee of $20. You will receive an invoice that is required to be paid before further placement of your child in the crèche.

COST: A charge of $5.00 per hour with 30min sessions is available (minimum booking times apply and include: 8.20am to 11.30am - minimum of 1 hour, 11.30am to 4.00pm - minimum 2 hours, followed by 30min increments. Last booking start time is 2.00pm.)

My signature confirms that I have completed all sections on this form and that in order to provide a safe, secure and happy environment for my child, I accept the following:

·  I have read and I understand the ECU Sports Crèche information flyer on our web page www.ecu.edu.au/sport/creche.

·  I agree to pay $5.00 per child per hour. A late pickup fee of $5 for the first 5 – 10 minutes I am late and $5 for every 5 minutes thereafter is applicable outside my booking time.

·  I will comply with the Government Regulations and I understand that cancellations need to be made in writing with at least one working days notice or a fee of $20 applies.
