Question 1

With a sample size of 20, failure time for the life expectance test of light bulbs is recorded as 4, 6, 18, 42, 50 (hours). Please calculate the following:

(a)Failure densityat Time interval 20-40 hr. [4 marks]

Question 2

ISO9000 Quality Audit

The following incident occurred during a third party audit of a producer of computer monitor. It contains situations for which non-conformity reports may be required. Examine the incident carefully and reference it with the BS EN ISO9001:2000 standard. Answer the questions and then fill in a non-conformity report when you think necessary.

In the storehouse, the auditor notices a box containing seven CRT tubes each with two labels attached; REJECT & URGENT. The Chief Inspector explains that this is part of a purchase order from a new supplier,‘twelve items were supplied three weeks late and when inspected these seven were found not to conform to the specification’. The auditor asks what happened to the other five CRTs. The Chief Inspector states that because the contract was urgent, five were released to production before the inspection could take place. In the production department, the Floor Manager explains that the five CRTs have been used to complete export orders. There is no record of which order used the suspected CRTs. In the Purchasing Department, the auditor asks how the new supplier of CRTs was selected. The purchasing clerk explains that the regular suppliers could not meet the delivery date and the order was placed with a new supplier because the price was extremely low. The clerk states that it was not a normal practice to consult or advise any other department in cases like this one.

7.4 Purchasing

7.4.1 Purchasing process

The organization shall ensure that purchased product conforms to specified purchase requirements. The type andextent of control applied to the supplier and the purchased product shall be dependent upon the effect of thepurchased product on subsequent product realization or the final product.

The organization shall evaluate and select suppliers based on their ability to supply product in accordance with theorganization's requirements. Criteria for selection, evaluation and re-evaluation shall be established. Records of theresults of evaluations and any necessary actions arising from the evaluation shall be maintained (see 4.2.4).

7.4.2 Purchasing information

Purchasing information shall describe the product to be purchased, including where appropriate

a) requirements for approval of product, procedures, processes and equipment,

b) requirements for qualification of personnel, and

c) quality management system requirements.

The organization shall ensure the adequacy of specified purchase requirements prior to their communication to the

supplier.

7.4.3 Verification of purchased product

The organization shall establish and implement the inspection or other activities necessary for ensuring thatpurchased product meets specified purchase requirements.Where the organization or its customer intends to perform verification at the supplier's premises, the organizationshall state the intended verification arrangements and method of product release in the purchasing information.
