14. General Summary of Function/Purpose of Position
Conducts physical and mental assessment of the patient formulating nursing diagnosis, interaction strategies, and contributing Nursing’s role to patient’s interdisciplinary care plan (IPOS). Administers medication. Follows documentation guidelines. Conducts socialization, psycho educational and group therapy sessions. Performs high risk assessments of patient/staff injury, suicides, seclusion and restraint assessments and readiness for release. Responsible for and oversees professional nursing care of patients. Responsible to maintain nursing competency. Provides emergency care to patients and staff as needed. Participates in department and center-wide activities to maintain JC accreditation efforts including patient safety, P.I., I.C. and other pertinent patient care activities.

Duty 1
General Summary of Duty 25% of Time
Conducts physical and mental assessment of patient formulating and nursing diagnosis, interaction strategies, and contributing Nursing’s role to patient’s interdisciplinary care plan (IPOS).
Individual tasks related to the duty.
1 -Conducts interview and gathers history to determine patient problems needs and nursing diagnosis.
2 -Interprets information to formulate nursing plan of service.
3 -Reports findings to interdisciplinary team.
4 -Utilize data obtained from assessment to formulate intervention plans and appropriately reassesses as
necessary.
5 -Collaborates with interdisciplinary team and the patient to develop the IPOS.
6 -Implements nursing care identified in IPOS.
7 -Participates in patient-centered planning
8 -Documents assessment date and interventions completely, effectively and timely per hospital, department
policies and guidelines.
9 –Demonstrates good clinical judgment in prioritizing good patient care.
Duty 2
General Summary of Duty 30% of Time
Administration of medication
Individual tasks related to the duty.
1 -Process physician’s orders
2 -Acquire medication from Pharmacy.
3 -Comply with JCAHO, DCH, hospital and department medication standards and policies during
medication administration process.
4 -Assess patient’s needs for and effectiveness of the medication and document such per procedure.
5 -Maintain nursing medication records.
6 -Observation of patient for side effects of medication.
7 -Maintain patient safety standards in regards to medication process.
8 -Reports medication errors.
Duty 3
General Summary of Duty 20% of Time
Organize, plan and implement patient orientated activities inclusive of physical health programs for
patients and social, psycho education, group therapy sessions for patients.
Individual tasks related to the duty.
1 -Design and implement physical health programs for patients.
2 -Design and implement group therapy sessions to help understand their symptoms and those of their
peers.
3 -Conduct groups to increase patient socialization skills.
4 -Conduct groups to increase patient’s functional skills.
5 -Conduct medication groups for patients.
6 -Document patient participation and progress per charting guidelines.
Duty 4
General Summary of Duty 25% of Time
Perform required nursing functions in clinical area.
Individual tasks related to the duty.
1 -Perform all nursing functions as stated in hospital policies, i.e., seclusion, restraints, etc.
2 –Perform all nursing shift duties as described in Nursing Procedure Manual.
3 -Administer CPR and FA as needed
4 -Monitor special physical problems of chronically ill patients.
5 -Do all treatment as prescribed by the physician.
6 -Documentation of nursing observations, interventions in accordance with charting guidelines.
7 -Maintain Nursing skills and competencies as described in Nursing Procedure Manual.
8 -Performs tasks related to Performance Improvement goals and government regulatory requirements of
CLIA.
9 -Maintains medical equipment and supplies within their control including monitoring use, usability, and
presence.
10-Assist in precepting of new nursing employees as required.
11-Participate in committees and workgroups as required.

Assess physical care and the patient needs including short and long term care. Patients are affected
by this decision. Ability to call for an ambulance. Patients are affected by this decision. Seclusion and
restraint, physical and mental health assessment and readiness for release which affects patient and unit
milieu.

Nursing decisions that fall outside of established procedures and policies.
Nursing competency is reviewed each year or as a need would arise.
18. What kind of physical effort do you use to perform your job? What environmental conditions are you physically exposed to on your job? Indicate the amount of time and intensity of each condition. Refer to instructions on page 2.
Physical activities of the job are standing, sitting, lifting up to 25 pounds. You require digital dexterity, repetitive use/movement of hands and ability to read small print to administer medications. The ability to work all three shifts and provide mandatory overtime coverage in emergencies. You must be able to perform CPR in an emergency. You need to be able to stand 50% of the time, climb stairs 10%, and sit 40% of the time without any ill effects. You need to be able to walk or run if the need arises. The nurse may be exposed daily to patient verbal and physical aggression, biohazardous waste, hazardous chemicals, and to needle exposure injuries.

23. What are the essential duties of this position?
Direct care of patients both physical and emotional. Carries out nursing duties on assigned unit.
Assess, document, integrate and implement individual nursing care plans for each patient. Performs
nursing duties as assigned by supervisor. Instructs in-service programs when indicated.
Work requirement – 8.0 hours/day or a minimum of 40 hours/week, must be available to work overtime

The organizational structure remains the same. The scope of the duties have increased in accordance
with the individual needs of the patients on the unit.
25. What is the function of the work area and how does this position fit into that function?
The Department of Community Health Center for Forensic Psychiatry is a high risk speciality hospital
accredited by JCAHO. The patients are mentally ill criminal offenders who can be dangerous to themselves
and others. They are pending psychiatric examinations and their behavior is unpredictable. This position
would have direct contact with the above mentioned patients on a daily basis.
26. In your opinion, what are the minimum education and experience qualifications needed to perform the essential functions of this position?
EDUCATION:
Graduation from an accredited School of Nursing.
EXPERIENCE:
No specific type or amount of experience is required if a master’s degree in Nursing is held OR
6 months of experience equivalent to a R.N. P11 in state service, in combination with a bachelor’s
Degree in Nursing OR One year of experience equivalent to a R.N. P11 in state service, in combination
With a diploma or an associate’s degree in Nursing.
KNOWLEDGE, SKILLS, AND ABILITIES:
Ability to independently organize and coordinate the professional registered nurse activities of a specific
work area. Ability to determine work priorities. Knowledge of general nursing principles and practices.
Knowledge of medications and treatments. Knowledge in the application of nursing techniques. Ability
to assess the effectiveness of nursing care. Ability to carry out instructions.
