INSTRUCTIONS :

j Type “To increase the number of hazards addressed by the improved school emergency response plan as compared to the baseline plan” under “Project Objective.”

k Type the REMS GPRA 1 statement in the “Performance Measure” box on Section A (Project Status Chart) of the ED 524B.

l Type “GPRA” in the “Measure Type” box.

m Under “Target/Ratio” indicate the total number of hazards you AIM to address in the numerator and the total number of hazards you CURRENTLY address (baseline) in the denominator. In this example, ABC School District currently addresses 10 hazards, but AIMS to address 12 hazards by the end of the grant period. For example, in this case:

12 = TARGET number of hazards to address at the end of the grant (Numerator)

10 = BASELINE number of hazards addressed at the beginning of the grant (Denominator)

n Under “Target/Actual Performance Data” indicate the TOTAL number of hazards you ACTUALLY address at the end of the project period in the numerator. In the denominator, indicate the baseline number of hazards you addressed at the beginning of the grant period. For example, in this case:

14 = ACTUAL total number of hazards addressed at the end of the grant (Numerator)

10 = BASELINE number of hazards addressed at the beginning of the grant (Denominator)

** NOTE: The denominator will be the same for both Target and Actual.

o Under the “Explanation of Progress” section, include the following:

Data:

a)  A narrative list of the Actual hazards addressed by district(s) (or school) baseline plan(s)

b)  Target number of hazards to be addressed by the improved plan(s) (the number of hazards you hope to address by the end of the grant period)

c)  A narrative list of the Actual TOTAL hazards addressed by the improved school emergency management plan(s) (at the end of the project period only)

Additional Information:

·  A narrative for the data provided, explaining what the numbers mean and how they were calculated.

·  An explanation of the rationale for the target amount of hazards addressed and when it was established. (ie, what additional hazards did you want to address during the grant period and why?)

·  A specific list of the hazards actually addressed in the final plan(s).

·  A description of any barriers that prevented you from meeting the target, if applicable.

·  A list of any unanticipated hazards that were also addressed during the grant period, but, may not have been part of the target list.

NOTE: If the number and type of hazards addressed in the district emergency management plan and in individual school emergency management plans are different, please provide two different sets of data—one for the district plan and one for school plans. Include both Actual and Target number of hazards for both sets of data.

n Explanation of Progress (Include Qualitative Data and Data Collection Information)

Our district plans to conduct training on Basic First Aid at all 10 of our district schools. Our first project-specific objective, as stated in our application, is “To train 5 crisis team members in each of our 10 school buildings in basic first aid as measured by completion of the “Basic First Aid Training” course offered by our local Red Cross.” If we successfully reach our target for this objective, we will have trained 50 staff by the end of the grant period. We will be able to measure our progress towards this objective by tracking the number of completion certificates received by our staff (we will photocopy them and maintain copies in the district records).

Thus far, our district has completed training at 5 of our 10 schools. At four of the five schools where training was conducted, we had perfect attendance. Unfortunately, at the fifth school, 2 of the potential trainees were sick during the day of training and therefore did not complete the required training. Therefore, we have only trained 23 of the 25 staff we hoped to train at the first five schools. Since we have five trainings left (for the remaining 5 schools) we will try to get the 2 staff who missed the initial training to sit in on an upcoming training so we can meet our ultimate target of 50 trained staff.

NOTE: We defined “trained staff members” as those staff that successfully completed the entire Basic First Aid Training Course (8 hours) and received a “Certificate of Completion” from our local American Red Cross chapter.

INSTRUCTIONS FOR REPORTING PROJECT-SPECIFIC OBJECTIVES ON THE ED 524B:

j  Type your project-specific objective in the “Performance Measure” box on Section A (Project Status Chart) of the ED 524B.

j  Type your performance measure (how you will assess this) in the “Performance Measure” box on Section A (Project Status Chart) of the ED 524B.

k  Type “PROJECT” in the “Measure type” box because this measure is one of your project-specific objectives (NOTE: This means it the objective is unique to your application and your grant project.)

l Under “Quantitative Data,” include the following:

·  You may either enter date in the Raw Number section, the Ratio Section, or the % section. You may chose whichever method is the easiest way to report on your particular objective. In the example above, the district chose to report on “raw numbers.”

·  Raw Number

o  Target: This is your target number for this objective -- it is what you hope to achieve through this objective. In this example, the district hopes to train 5 staff each at 10 schools (or 5 *10 = 50 staff). Therefore, their raw number target is: 50 trained staff members.

o  Actual Performance Data: This is the actual number for this objective at the time of the report -- what you have actually done. In this example, the district has conducted 4 trainings with 5 staff per training and 1 training with only 3 staff in attendance ([4*5] + [3]) = 23 staff). Therefore, their raw actual performance number is 23 trained staff.

m Under the “Explanation of Progress” section, include a narrative explanation of your progress towards accomplishing the objective. This explanation should include the following:

·  A detailed explanation of the numbers you indicated in the quantitative section for this objective.

·  An explanation of any challenges that may have prevented your project from reaching your target.

·  The district(s) plans for reaching the target in the remaining days of the current project period.
