SUPPORTING DOCUMENT FORM

Thank you for booking one of our access ticket types for BST Hyde Park 2018.

Proof of disability is required to collect your PA ticket and/or entrance to your booked place on the Viewing Platform or Ground Level Viewing Area.

This year we have revised our system to ensure you have the opportunity to submit the supporting documents in advance.

The closing date for submitting evidence in advance is Friday 8th June 2018 (four weeks prior to the first show). If you have not submitted evidence by this date you will be required to show evidence once you reach the event.

Submitting supporting documents

When submitting documents (details of which are provided below), we ask that you:

·  Scan evidence and attach it with your form if emailing

·  Photocopy evidence and clip it to a printed form if posting
