·  Portiuncula Hospital is a teaching hospital affiliated with the National University of Ireland Galway has 203 beds and provides a range of services to a population of approx. 400,000 patients residing in the East Galway, Roscommon, Midlands and Midwestern areas.
·  Portiuncula has a directorate management structure established under the Office for Health Management Clinicians in Management Initiative.

CNM3 in Emergency Department
Louth Meath Hospital Group- Our Lady of Lourdes, Drogheda (a panel will formed for all Louth Meath Hospital Group).

CNM3 Bed Management
1 X WTE in Cavan General Hospital (a panel will be created for all Cavan Monaghan Hospital Group).
Cavan and Monaghan Hospital comprises of two hospital sites, Cavan General Hospital and Monaghan Hospital with services operationalised as one hospital on two sites. Both hospitals deliver a wide range of services to the Cavan Monaghan population (approx 133,369 Census 2010) which includes assessment, diagnosis, treatment and rehabilitation of both acute and complex conditions as well as non-urgent/ non-acute conditions. The inter-dependency in the roles of the hospitals strives to provide a seamless patient service providing timely and appropriate care in the most appropriate setting. In 2012 there were approximately 14,787 discharges and over 40,000 attendances.
Both hospitals have a key role in providing both acute and non-acute hospital services to the Cavan and Monaghan population which include a range of services. For example (not an exhaustive list):

CNM3 Patient Flow Cancer Services
Galway University Hospitals
1 X Whole Time Equivalent. A panel will be created for Galway University Hospitals.
The successful applicant will:
·  Co –ordinate the flow of patients from the Emergency Department (ED), the Acute Medicine Unit (as per policy), Cancer Day service areas and OPD to appropriate cancer service inpatient wards or as agreed with the Bed Flow Team.
·  Develop close linkages with specialist units (hospice inpatient ward), homecare teams and community services
·  Communicate with patients and family in relation to current bed status; liaise with Bed Flow Team and Bed Management as appropriate to expedite the situation. Controlling the utilisation of patient accommodation in adherence with the Admission Policy and Integrated Discharge Planning Process.
·  Maintain close communication links with Bed Flow Team, Bed Management, the Clinical Directors, the Medical Consultant teams, the Chief Operation Officer, the Waiting List office, DON/M, ADON’ s, CNM’s, liaison public health nurses and relevant specialist services (AHP’s, Transport services).
·  Maintain a close working relationship with all identified above and all other relevant specialty clinicians and nursing staff in relation to senior decision making around the allocation of patients to beds, incorporating the elective cancer activity cases for admission.
·  Link with the discharge co-ordinator to plan for patients that have been identified requiring longer term care/ respite.
·  Where bed occupancy has reached a predefined level, it will be the responsibility of the Patient Flow Coordinator in collaboration with the management team to implement the Galway University Hospitals escalation plan to meet demand.
·  The Patient Flow Coordinator will also direct patient flow from this hospital to other hospitals within and outside the group as clinically appropriate for sub-acute care or step-down care, when the acute episode of patient care has been completed.
·  Develop and implement an agreed strategy to co-ordinate direct access for patients to the CAU from GPs after phone triage.
·  Participate in the roll out of the Electronic patient record and scheduling tools on Lantis or upgraded version. Integration with other IT data base systems as pertain to cancer services, MDM software, Dendrite, NCCP KPI’s
KPI’s
·  The identification and development of Key Performance Indicators (KPIs) which are congruent with the Hospital’s service plan targets.
·  The development of Action Plans to address KPI targets.
·  Driving and promoting a Performance Management culture.
·  In conjunction with line manager assist in the development of a Performance Management system for your profession.
·  The management and delivery of KPIs as a routine and core business objective.
The post holder will ensure the clinical and operational effectiveness of cancer services within Galway University Hospitals, develop close linkages with territory cancer service units across the group and play a role in the implementation of the Acute Medicine Programme, emergency medicine programme and Cancer control plan.

Purpose of the Post / To provide professional / clinical leadership in the designated area(s) of responsibility. To oversee the management of resources including staffing and staff development. To facilitate communication across the healthcare teams.
The post of CNM 3 has a pivotal role in service planning and development, co-ordinating and managing activity and resources within the clinical area. The CNM 3’s responsibilities include: overseeing the quality of nursing care, the development and implementation of policy and procedures, the monitoring of activity and the delivery of agreed levels of service for the designated area(s). The CNM3 plays a key role in providing clinical and professional leadership and promoting the values of the organisation.
Principal Duties and Responsibilities / Professional /Clinical
The CNM3 will:
·  Provide a high level of professional and clinical leadership
·  Provide safe, comprehensive nursing care to service users within the guidelines laid out by An Bord Altranais
·  The Manager will practice nursing according to
Professional Clinical Guidelines
National and Area Health Service Executive (HSE guidelines).

The CNM 3 will:
·  Ensure that effective safety procedures are developed and managed to comply with statutory obligations, in conjunction with relevant staff e.g. health and safety procedures, emergency procedures
·  Take appropriate action on any matter identified as being detrimental to staff and/or service user care or well being / may be inhibiting the efficient provision of care
·  Ensure adherence to established policies and procedures e.g. health and safety, infection control, storage and use of controlled drugs etc.
·  Ensure completion of incident / near miss forms
·  Maintain a feedback mechanism with the clinical risk manager and report to senior management team where appropriate
·  Ensure adherence to department policies in relation to the care and safety of any equipment supplied for the fulfilment of duty. Ensure advice of relevant stakeholders is sought prior to procurement e.g. CNS infection control, Occupational Therapist
Education and Training
The CNM 3 will:
·  Contribute to service development through appropriate continuous education, research initiatives, keeping up to date with nursing literature, recent nursing research and new developments in nursing management, education and practice and attend staff study days as considered appropriate
·  Provide support advice to those engaging in continuous professional development in his / her area of responsibility
·  Be familiar with the curriculum training programme for student nurses and be aware of the clinical experience required to meet the needs of the programme
·  Participate in the identification, development and delivery of induction, education, training and development programmes for nursing and non-nursing staff
·  Provide support supervision and professional development of appropriate staff
·  Engage in performance review processes including personal development planning e.g. by setting own and staff objectives and providing and receiving feedback
Management
The CNM3 will:
·  Exercise authority and co-ordinate the functions of the assigned area(s)
