CRADA QUESTIONNAIRE FOR THE VENDOR (COLLABORATOR)

BACKGROUND

The Joint Interoperability Test Command (JITC) Fort George G. Meade (FGGM) was designated by the Joint Chiefs of Staff as the authority that certifies whether critical information systems used by the Department of Defense (DoD) meet interoperability requirements for joint military operations. JITC FGGM, which is an organizational element of the Defense Information Systems Agency (DISA), has provided on-site technical support, throughout the past three decades during military exercises and contingency operations.

JITC’s FGGMability to provide support to DISA and the warfighter is characterized by several unique roles. JITC FGGMis the Operational Test Agency (OTA) for DISA-managed programs, but also serves as the OTA for other DoD Agencies such as the Defense Logistics Agency, the Defense Finance and Accounting Service, and the Defense Commissary Agency. JITC FGGMalso serves as the Executive Agent for specific testing needs of the Federal Emergency Management Agency (FEMA), the National Security Agency (NSA), and the National Geo-Spatial Intelligence Agency (NGA).

As a non-Service element of the Major Range and Test Facility Base (MRTFB), JITC FGGM deals directly with vendors to test and certify their products on a reimbursable basis. This early involvement usually results in more effective systems at lower costs.

In accordance with the DoD Directive (DoDD) 4630.5 and Chairman of the Joint Chiefs of Staff Instruction (CJCSI) 6212.01D, “All Information Technology (IT) systems and National Security Systems (NSS) must be evaluated and certified for Joint interoperability by JITC FGGM.” CJCSI 6212.01D further states that DoD Standards Conformance/Performance Certifications, Interoperability Assessments, and Information Assurance Assessments must be accomplished as a part of the overall Joint Interoperability Program.

When executing these types of assessments and certifications of Commercial products, JITC FGGM establishes a Cooperative Research & Development Agreement (CRADA) with the vendor. As a legal binding document, the CRADA allows JITC FGGM to serve as the vendor’s “trusted agent” during all phases of planning, test execution, and test reporting. JITC FGGM executes their CRADA authority through United States Code Title 15, Chapter 63, Section 3710a. As an element of the MRTFB, JITC FGGM is also subjected to all provisions of the DODD 3200.11 (MajorRange and Test Facility Base) and DODD 7000.14-R (DoD Financial Management Regulation).

QUESTIONNAIRE

This questionnaire is be used by the JITC FGGMAction Officer to acquire information from the vendor for insertion into the CRADA. Once the information is obtained from the vendor (collaborator), the assigned JITC FGGMAction Officer will complete the CRADA and forward it to the vendor for signature. The CRADA will be sent to the vendor in Adobe format. NO modifications to the CRADA template can be made by the vendor in accordance DISA/JITC FGGMpolicy and instructions. The following information is required:

Provide the name of the State the vendor’s corporation is located. Also, identify whether or not vendor is a foreign-owned company or a subsidiary of a foreign owned company.

NOTE: The JITC FGGMAction Officer will forward the CRADA to the DISA General Counselfor approval ONLY if the vendor (collaborator) is a foreign-owned company. CRADAs involvinga foreign-owned company must also be reviewed and approved by the United States Trade Representative (USTR).
