Instrument Contract
I______, will be using the following instrument______from______to ______. If I should neglect, lose, damage or willfully destruct this instrument, I agree to pay the necessary fee to repair and/or replace this instrument. I agree that I am solely responsible for this instrument. Therefore, I will be responsible for any damage or loss to this instrument caused by any relative, friend, or any other person. Since I am a minor, I also realize that if there is any expense for my assigned instrument because of negligence, abuse, or loss, the cost of the repair or replacement of the assigned or used instrument mentioned below, will have to be met by parent(s) and/or guardian(s).
This is not a bill of any sort. There is no cost to use school instruments. This is simply a contract to ensure that in case of any intentional damage, neglect or loss caused by the student, relative, friend, or any other person, the cost to repair or replace the instrument will be met by the student’s parent(s) and/or guardian(s).

Contrato Para Instrumento Musical
Yo______, estare usando el siguiente instrumento musical ______de ______a ______. Si yo dano, pierdo, descuido, o destrozo este instrumento por mi propia voluntad, yo estoy de acuerdo a pagar los gastos para reparar or reemplazar este instrumento. Yo entiendo que yo soy el unico responsible de este instrumento musical. Yo sere responsible por cualquier dano or perdida causada a este instrumento por cualquier familiar, amigo(a), o cualquier otra persona. Siendo asi, que yo soy menor de edad, yo realizo que si hay cualquier gasto del instrumento assignado a mi por mi descuido, abuso, o perdida, el costo de reparacion o para reemplazar el instrumento assignado o el instrumento usado que se menciona abajo, tendra que ser pagado por mis padres o tutores.
Esto no es ningun cobro; no hay cobro por usar los instrumentos de la escuela. Solo es un contrato para asegurar que el instrumento que va a ser usado por el estudiante se va a usar con cuidado y que los gastos para reparar or reemplazar el instrumento sera cubiertos por los padres/tutores cuando los danos sean causados intensionalmente.
