6) BRIEF RESUME OF INTENDED WORK:

6.1 Need for the study:

Vatakantaka is a disease explained by Sushruta in the context of vatavyadhi as a painful condition of heel caused by its improper placement of foot on the ground1.

Pain beneath the heel is seen in infra calcaneum bursitis and plantar fasciitis2. Plantar fasciitis is a common cause for pain in the heel region, which occurs in about 10% of the general population and represents 10–15% of foot problems requiring professional care . Women are twice as likely as men to get plantar fasciitis3.Treatment modalities for Plantar Fascitis in contemporary system of medicine includes- placing a soft pad under the tender heel. Other treatments include Analgesics, NSAIDS and steroid injections at the tender part which provide temporary relief4.

Swedana is indicated in Vatakantaka according to Caraka5 and Chakrapanidatta has indicated Valuka Sweda in Vata and Kaphaja vyadhi6. The treatment for Vata kantaka also includes Raktavasechana, Eranda tailapana and Agnikarma as mentioned in classics7. Gandharvahasta taila mentioned in Ashtanga Sangraha is told to be the same as Eranda taila according to Indu Commentary8 which is the main ingredient in Gandharvahastadi taila. Hence the present study is selected to evaluate the efficacy of Valuka Sweda and Gandharvahastadi Taila in Vatakantaka.

·  Vatakantaka Nidanas & lakshanas is mentioned in Sushruta Samhita & Astanga Hrudaya1,9.

·  Reference of Eranda taila pana in the management of Vatakantaka is mentioned in Chakradatta.

·  Gandharvahastadi Taila mentioned in Ashtanga Sangraha.

·  Valuka Sweda explained in Chakradatta.

·  Pinda Taila mentioned in Astanga Hrudaya10.

6.3) Objectives of the study:

·  To evaluate the effect of Valuka Sweda in the management of Vatakantaka.

·  To evaluate the effect of Valuka Sweda with Gandharvahastadi Taila Pana in the management of Vatakantaka.

·  To compare both groups and evaluate the effect of Gandharvahastadi Taila Pana in the management of Vatakantaka.

7) MATERIALS AND METHODS:-

7.1) Source of data:

A minimum of 40 patients with Vatakantaka coming under the inclusion criteria will be selected for the study from the OPD and IPD of SKAMCH RC, Bangalore-104.

7.2) Method of collection of data:

·  It is a comparative clinical study with pre-test and post-test design, where in 40 patients of either sex, diagnosed as Vatakantaka, will be randomly assigned into two groups, each comprising of 20 patients.

·  A case proforma will be specially designed and duly filled with all points of history-taking, physical signs & lab investigations.

·  The parameters of signs and symptoms will be scored as mentioned in the proforma.

·  The data obtained in both groups will be recorded, tabulated & statistically analysed using suitable statistical methods.

Diagnostic criteria:

·  Patients suffering with lakshanas of Vatakantaka.

Inclusion criteria:

·  Patients presenting with lakshanas of Vatakantaka.

·  Patients of either sex, above 18 years.

Exclusion criteria:

·  Other systemic diseases which interfere with the course of treatment.

·  Patients with the Fracture and dislocation of ankle joint.

Study design:

A comparative clinical study with pre-test and post-test design will be conducted on 40 patients with lakshanas of Vatakantaka.

Intervention:

40 patients of Vatakantaka will be selected and randomly divided into two equal groups, as Group-A and Group-B.

Group-A- will be subjected to Sthanika Abhyanga with Pinda Taila followed by Valuka Sweda till Samyak swinna lakshanas are seen for 7 consecutive days.

Group-B- will be subjected to Sthanika Abhyanga with Pinda Taila followed by Valuka Sweda till Samyak swinna lakshanas are seen along with Gandharvahastadi taila pana(10 ml) in the early morning on empty stomach with warm water for 7 consecutive days.

Course of Treatment – 7 consecutive days.

Assessment will be done on:

Pre test – 1st day, Post test – 7th day, 1st follow up – 14th day.

Assessment criteria:

The assessment will be done on the basis of Subjective and Objective parameters as per the proforma.
