Performance Appraisal Manual
Instruction: Carefully evaluate employee’s performance in relation to current job requirements. Check rating box to indicate the employee’s performance that close to the prescribed descriptions. Decide score within the prescribed range of the grade and put in the right most column in the appraisal form .

1. Quantity of
work done
(Compare to quantity standard or expectation of the position /

2. Quality of
work / Extremely high quality of work / High quality / Generally are acceptable / Have to correct the work sometimes / Always have to correct the work

Following the procedures and instructions / Strictly follows the concept and intention of the procedures or instructions, convince others to follow too / Not only follow the procedures and instructions but also try to follow its concept and intention / Clearly follow the procedures and instructions / Not follow the instruction, have to get warning sometimes / Always neglects to follow procedures or instructions and cause problem to plan schedule
4. Human relations for work effectiveness / Very effective to contact other people even in case of the complicate situation / Good human relation with any level of people / Acceptable human relation with other / Seldom to have human relation problem with other people / Good relation to someone but always be prejudice and blame other people
5. Skill and level of understanding in work / Very skill and good understanding in work. Could solve the problem without guidance or instruction / Above average of understanding in work. 6. Work attendance / Always report to work on time, seldom to take leave and never absent without notice / Report to work on time regularly, taking leave only for necessary reason, easy to find him/her / Late for sometimes, acceptable number of leaves, give prior notice when going out of work / Late in work but still be in acceptable level, spend more leaves especially sick leave and disappears from work sometimes / Always be late or absent without prior notice, disappear from work regularly, take a lot of sick leave and absent

7. Ability of decision making / Clearly show a very good ability of decision making / Always make a good decision. Having ability to forecast situation / Generally could make acceptable decision / Always have problem due to lacking of knowledge but still could solve them / Always make unreasonable decision or never make decision
8. Initiative ability / Alert to improve and always make initiative for better way to work / Clarify the existing procedures and propose initiative of practical measurement / Making question to clarify procedure only when facing problem and propose alternative way sometimes / Just follow the existing procedure, seldom make initiative idea / Never make any initiative
9. Ability of leadership / Perform a very good directing, advise and controlling staff, be a good sample to subordinates / Effective leader, staff under control are always happy to work with him / Good at directing and give clearly instruction to staff. After evaluate all of the factors and decide score for each factors, you could sum and get total score which you can compare to the score guidance in order to give overall appraisal result.
