***Please attach a narrative describing yourself and your future plans. You may want to include activities in and out of school, summer activities or jobs, full-time jobs, special interests, and talents: be SURE to include your goals. This may not exceed two pages. Essay must be formatted using double spacing and a Times New Roman font, size 12 point.

***Please attach a response to one of the following prompts. This short essay should be no less than 500 words in length.

(1)  Why does education matter in your community?

(2)  What traits do you have that will enhance your education and future employment?

(3)  Describe the solutions you would propose to improve or resolve a current issue such as health care, the economy or discrimination.

***References: Please provide two (2) letters of recommendation from people who have been instrumental in your academic, church, or personal growth. Your recommenders may not be related (immediate or extended family) to you.

***For each scholarship for which you are applying, please attach a copy of your transcripts with grades and test scores along with all of the documents mentioned above: your narrative and short answer essay, a resume, the two letters of recommendation, any college/post secondary institution acceptance letters, and a copy of this completed application.
