Individual Waiver Form

(To be used by groups with participants

who are 16 or older.)

Assumption of Risk and Damages

To be read before renting a boat

I agree…

1. That I am 16 years of age or older.
2. Not to sublet or loan the boat.
3. To hold Deer Lake Boat Rentals, Silent Waters Boat Rentals and Silent Waters Ltd. harmless against any and all damages and claims against them however rising or occasioned, whether to property or to persons, including myself, anyone in my party and any third party.
4. That all passengers will wear personal floatation devices at all times.
5. That I am qualified and competent to operate and navigate the boat and will operate them in a careful and prudent manner at all times during the term of this agreement.
6. To return the boats and equipment in as good condition as when rented, excepting ordinary wear and tear and to pay for any and all loss of or damage to the boats and to pay for any and all expenses incurred in repairing any such damage, replacing any such loss, and if necessary obtaining the return of the boats or other property, as well as any revenues as a result of such damage or loss.
7. That I take full responsibility for my boat and all equipment rented by me and people in my boat and that all members in my boat are made aware of this waiver and the risks associated with this activity.
8. To follow the directions of Deer Lake staff and permit to be in tow and return to dock, any vessel where boat, or other properties, or passengers are deemed in danger.
9. That I am renting the boat and equipment at my own risk, and that I have read and understand this agreement.

Parent/Guardian Waiver Form

(This form is used for parents/guardians with participating children under 16)

Assumption of Risk and Damages

To be read before renting boats for your child or children

I agree…

1. That I am the parent and/or legal guardian of a child or children who will be participating in a boating activity at Deer Lake.
2. That I grant permission for my child or children to participate in a boating activity at Deer Lake and understand that there are inherent risks involved in any boating activity.
3. That I am 19 years of age or older.
4. To hold Deer Lake Boat Rentals, Silent Waters Boat Rentals and Silent Waters Ltd. harmless against any and all damages and claims against them however rising or occasioned, whether to property or to persons, including myself, anyone in my party and any third party.
5. That my child or children will wear personal floatation devices at all times provided by Deer Lake Boat Rentals.
6. That my child or children are qualified and competent to operate and navigate the boats and will operate them in a careful and prudent manner at all times during the term of this agreement. If they are not qualified and competent to operate and navigate the boats, then they will ask and follow directions from Deer Lake staff on how to operate and navigate the boats.
7. Group Waiver Form

(This form can be filled out by a group leader and can replace all

of the Parent/Guardian Waivers and all the Individual Waiver Forms)

Assumption of Risk and Damages

To be read before renting boats for your group

I agree…

1. That I am a designated group leader and I have the authority to sign on behalf of my group.
2. That the members of my group (or their parents and/or legal guardians) have granted me the authority to lead this group on this boating activity.
3. That I am 16 years of age or older.
4. That I will inform my group that it is their responsibility for the safety and security of their personal belongings including electronic equipment which may be damaged due to accidental water contact.
5. To hold Deer Lake Boat Rentals, Silent Waters Boat Rentals and Silent Waters Ltd. harmless against any and all damages and claims against them however rising or occasioned, whether to property or to persons, including myself, anyone in my party and any third party.
6. That all passengers will wear personal floatation devices at all times.
7. That my group and I are qualified and competent to operate and navigate the boats and will operate them in a careful and prudent manner at all times during the term of this agreement.
