Purpose of Job / To act as a Case Manager for adult service users entering structured treatment through the Criminal Justice System, enabling service users to address their substance use, offending behaviour and plan their own personal recovery plan. The treatment strand of delivery is for service users who are unlikely to, or have been unable to, substantially address their substance use issues and achieve abstinence within 3 months as part of the ReNew early engagement offer.
The post holder will work holistically tosupport service users on their caseload to resolve all identified needs essential to long-term recovery including support to address: substance use, physical and mental health needs, employment, training and social needs, family relationships and personal well-being, debt and housing needs.
The role includes harm reduction and assertive engagement for those active in the criminal justice system but struggling to engage with services; and delivering compulsory interventions for service users who are subject to bail, licence or court conditions with the aim of reducing offending, reducingharm and increasing motivation to change.
There is a wide-range of group work programmes available to ReNew service users as part of structured treatment; as well as Medical and Healthcare interventions. It is part of the case management function to enable service users to develop a Recovery Plan that incorporates the most appropriate treatment options and facilitate access to these.
Aftercare is an important part of theReNew Service, Case Managers will work with the Housing, ETE, and BRIC Workers to assist the service user to establish adequate recovery capital to sustain change, such as suitable housing, family and social networks, employment, training or education and meaningful activity according to their personal goals.
Key Activities /
  • Engaging with people who have substance misuse problems and providing them with advice on drug/alcohol issues, health issues and social functioning.This includes Harm Reduction information and discreet needle exchange where appropriate.
  • To promote healthier life styles and increase service user choice, efficacy and improve well-being
  • To promote family, carer and service user involvement within the service user’s individual recovery plan
  • Effectively assess, plan and review all stages of a service user’s treatment journey using designated tools and within agreed timescales; enable service users to develop individual recovery plans that clearly identify how achieving each goal will enable progression in addressing substance use, reducing reoffending and reintegrating into the community.
  • Assist and carry out appropriate screening processes including urine and saliva tests.
  • To promote and support individuals into both abstinence based and medically assisted forms of recovery
  • To work in partnership with other agencies to provide a package of support around holistic needs, by attending joint case allocation and case review meetings, sharing appropriate information and ensuring that action plans are carried through.
  • Making Contact with Service Users prior to release from Prison and ensuring a comprehensive release plan is in place.
  • To utilise motivational interviewing, ITEP, cognitive behavioural techniques and brief interventions in both one-to-one and group settings to promote engagement in treatment services, encourage self-esteem, well-being, self-responsibility and enhanced motivation.

  • Undertaking assessments for Tier 4 inpatient detox and/or residential rehab, presenting the case to panel and supporting the service user through the tier 4 pathway.
  • Supporting Medically Assisted Recovery by facilitating and attending medical assessments and reviews with the clinical team, issuing prescriptions according to CRI Policy and Procedures and locally agreed protocols between the case management and clinical teams, and referring to the Health and Well-being Nurses for health interventions.
  • To actively refer to a range of community support agencies facilitating access to practical support.
  • Employing assertive engagementapproaches in partnership with Police Officers and other relevant service providers to maximise the potential engagement of those who have been identified as entrenched substance misusers and prolific offendersto maximise the potential engagement of these service users.
  • Supporting service users with employment, training and education goals, building family and/or positive social relationships and to utilise mutual aid, as appropriate to the individual.
  • Undertaking other elements of general service delivery as required, including Reception, Duty Cover, and 24/7 Helpline.
  • To maintain accurate and comprehensive records on clients on the clinical/case management database and complete paperwork with service users to a high standard.
  • To undertake any administrative function related to the role to a high standard, in a competent and timely fashion.
  • Whilst ReNew Criminal Justice is committed to offering clearly defined delivery strands, the Early Interventions Case Management Team and Treatment Case Management Team may be required to cover each other’s functions where service need dictates.

General terms of reference. /
  • You will be required to work flexibly across operational sites as required and work flexibly within an agreed number of hours to maintain the most appropriate level of service provision. This will include evening and weekend working.

  • Keep abreast of developments in services, legislation and practice relevant to the post.
  • Ensure that all activities, and interventions are suitably recorded and in accordance with CRI policies, procedures and national guidelines from the NTA, DoH, NICE, NMC, CQC and as directed by the local management team
  • Ensure that all safeguarding issues, including child protection and protection of vulnerable adults, are dealt with in accordance to CRI policies and procedures, and local and national statutory requirements.

Person Specification.

Essential criteria:

  • A valid UK driving licence.
  • Computer literacy

Experience of :

working with individuals that use substances, and delivering high quality services with demonstrable outcomes for this service user group.

Experience of undertaking a comprehensive assessment of need and enabling the service user to develop a personal Recovery Plan aimed at addressing these needs and achieving their goals.

Using a wide range of techniques such as ITEP, Brief solution Focused Therapy, Motivational Interviewing or Cognitive Behavioural Therapy to enable service users to overcome barriers and achieve recovery.

Sound and demonstrable knowledge of clinical prescribing issues and the ability to know when to seek support from a clinical member of the team.

A good working knowledge of:

 Substance use, offending and the issues facing substance misusers including housing, social care, physical and mental health needs, personal and family needs, employment, education and training needs.

Relevant Law and Best Practice, particularly in relation to Safeguarding Children and Vulnerable Adults and Data Protection
