1. Protects individuals from abuse or neglect at all times and immediately reports
any concerns.
2. Talks with individuals politely and positively during all activities.
3. Looks for possible injuries, seizures, changes in behavior that may mean a
health problem, reports immediately, ensuring medical attention, & documents.
4. Follows daily routine/classroom schedule all the time notifying supervisor if
there is a conflict.
5. Assists individuals in using and maintaining all adaptive equipment including
gait belts and walkers safely. Uses lifts safely, if applicable. Includes but not
limited to eye glasses, hearing aides, etc.
6. Dresses or assists in dressing assigned individual in his/her own clothing that is
clean and neat, and fits. Changes or ensures changing of anyone who is not.
7. Makes sure all individuals are clean, well groomed, and neat at all times(face &
hands clean, skin care done, hair clean and styled, teeth clean, breath fresh and
nicely shaven).
8. Provides care including bathing, teeth brushing, shaving, hair care, menstrual care, etc. if the individual cannot and does so protecting privacy and showing respect for personal preferences.
9. Does Eating Programs (and documents) or feeds individuals making sure person and area are neat and clean during meal, talking to individuals, and making mealtime pleasant.
10. Follows menus and diets correctly encouraging healthy eating habits and observing for and documenting individual preferences
11. Does group activities as assigned, making sure all individuals are involved andare enjoying activity. Uses praise and encouragement for participation.
12. Conducts individual self-care programs consistently and correctly and documents immediately. Praises successful performance or attempts.

33. Remains on home. Monitors and deals with any issues regarding staff breaks and staff time away from home.
34. Keeps new staff with him/her for first week of on-home orientation, teaching skills and observing performance of those skills.
35. Continues training of new staff (after first week) making sure all tasks are trained before new staff does them alone with individuals. Delegates training to experienced staff as appropriate
36. Trains existing staff on new programs/ procedures.
37. Meets regularly with all staff to make sure everyone is updated on all individual needs. Conducts positive and productive meetings including self-scheduling meetings.
38. Makes staff assignments. Makes sure all are following daily routines and safety procedures.
39. Completes staff evaluations as scheduled with input of supervisor.
40. Addresses performance issues fairly and as they occur. In consultation with supervisor, issues disciplinary actions as needed. Rechecks issues, giving feedback to staff as needed and documents.
41. Communicates well and shares information with other shifts. Helps out other shifts whenever possible.
42. Reviews program and other required documentation and retrains staff as needed.
43. Completes program observations and quality assurance checks as required using them as a tool in staff evaluations
44. Keeps supervisor and QMRP informed of all issues affecting the operation of the home or the well-being of the individuals served.
