Overall – An average of all questions with a 4 point (Always to Never) range.

Timeliness – Questions: 5, 6, 8, 10 and 11.

Thoroughness – Questions: 12, 13, 14, 15, 20, 22, 26 and 29.

Communications – Questions: 18, 21 and 24

Friendliness – Questions: 30, 31, 32, and 33.
