1.  In these states, you get:

g  Savings — Negotiated discounts that result in significant cost savings for you when you visit in-network providers, helping you to maximize your benefits. A First Choice Health logo on your health insurance card tells both you and your provider that a First Choice Health discount applies.

2.  PHCS Healthy Directions, which extends your local network nationally:

g  Giving you access to more than 4,500 hospitals, 75,000 ancillary care facilities and 700,000 healthcare professionals throughout the country.

g  A PHCS logo on your health insurance ID card tells you and your provider that a PHCS discount applies.

g  Your out-of-pocket costs will be based on your plan’s in-network coinsurance levels.

How to find participating providers

We can help you find the provider of your choice. Simply call [888-636-7427] Monday through Friday from 8 a.m. to 8 p.m. (Eastern Standard Time) and identify yourself as a health plan participant accessing the First Choice Health and PHCS Healthy Directions Networks. You may also search online at www.multiplan.com:

g  Click on the “Search for a Doctor or Facility” button

g  In the front of card section, choose the “Other network logos” tab
and select the logo shown here:

g  If you are searching for a provider outside Washington, Idaho
or Montana, choose the “Most common logos” tab and
check the box that says” PHCS Healthy Directions or Extended PPO”

g  Follow the prompts to enter your search criteria.

Before your appointment

It is your responsibility to confirm your providers’ continued participation in the First Choice Health and PHCS Healthy Directions Networks and accessibility under your benefit plan. Please also be sure to follow any required preauthorization procedures (usually a telephone number on your ID card), and always present your benefits ID card upon arrival at your appointment.
