2011-12 SCHOLARSHIP PROGRAM APPLICATION FORM

The ACTEAZScholarship applicant shall be a student who is enrolled in a recognized career and technical education program at the high school (from Funding Priority List) or a recognized career and technical education program at the post-secondary level (may include CTE teacher preparation program). A member of ACTEAZwho teaches in the program in which the student is enrolled or another member must recommend the applicant. The award(s) will be payable tothe educational institution identified by the recipient(s). The award amount can range from $500.00 – $2,500.00 based upon the available fundsand the number of scholarships the committee agrees to award. ACTEAZreserves the right not to award scholarships or to award scholarships in a lesser or greater amount in any given year. The ACTEAZ scholarship is non-renewable (meaning this scholarship is a one-time award).

Recognition will be given at the ACTEAZ Awards Session during the Annual Conference.

The following constitutes the complete application packet. All materials must be included and sent to ACTEAZ at one time.

Completed application form (ALL MATERIALS MUST BE TYPEWRITTEN ORWORD PROCESSED EXCEPT TRANSCRIPTS AND SIGNATURES. NO HANDWRITTEN INFORMATION IS ACCEPTABLE.)

Unofficial Transcript (High School must show previous five semesters; Post-Secondary/University must show grades for at least 12 semester credits). Courses listed in section III/A of the application should be highlighted on the transcript.

The nominating ACTEAZ member should review the completed application packet for accuracy, details, and completeness prior to submission and to ensure that application is word processed. Incomplete application packets will not be considered. If any of the information is considered to be false, the application will be disqualified. Decisions of the Scholarship Committee are final. ADDITIONAL TYPED/WORD PROCESSED PAGES MAY BE ATTACHED.

Specific Qualifications:

  • High School: Must be a graduating senior with enrollment in a CTE program on the fundingpriority list.
  • Post-Secondary/University: Must have completed at least 12 semester credits
  • Preference will be given to applicants who are dues-paid members of and actively involved in a career and technical education student organization(s)(CTSO: DECA/Delta Epsilon Chi, FBLA/PBL, FEA/Phi Delta Kappa, FFA, FCCLA, HOSA , Skills USA)
  • A member of ACTEAZmust recommend each applicant.An ACTEAZ member may recommend a maximum of two students.

Selection Criteria: CTE Involvement, CTSO involvement, Letter of Interest, Résumé, Letter of Recommendation, and Character Reference Letter. (See Application Form for point values.)

The letter of interest that you submit is a critical component of your application. Letters that are not well written, include spelling and/or grammatical errors, or lack substance will impact the review of your file. It is expected that you will provide a professional, well written letter that is approximately one page in length (500 words maximum). In the letter you should provide information to the Committee explaining why you are attending college, listing your career goals and aspirations, and describing how a monetary award from the ACTEAZScholarship for college expenses will help you attain your goals.(Please attach to this document)

VII. LETTERS(30 points)

Letter of Recommendation from the nominatingACTEAZ member which includes a detailed description of student’s involvement in classroom and laboratory, CTSO, work based learning experiences, and advisory councils. Maximum two pageswritten on official letterhead from the author’s organization; please attach to this document.

Character Reference Letter from a non-relative adult (teacher, counselor, administrator, clergy, supervisor, community member, etc.) which includes a description of the relationship between the student and author and their assessment of the student's strengths. Maximum one page written on official letterhead from the author’s organization (as appropriate); please attach to this document.

D.Release of information

By asking to be considered for an ACTEAZscholarship you are giving your consent for ACTEAZ to release your personal information to ACTEAZ Affiliates who will be considering you for scholarships.
