I wish to apply for the provision of services for ante mortem and post mortem inspection of animals slaughtered inKepak, Drumquin, Tiermaclane, Ennis, Co. Clare.

I certify that:

  • I have the necessary knowledge and understanding of the Hygiene Package;
  • I am willing to comply with all Health and Safety requirements;
  • If selected I expect to be generally available for offers of work in Kepak, Drumquin, Tiermaclane, Ennis, Co. Clare;
  • I will comply with all reasonable DAFM directions as they relate to provision of ante mortem and post mortem meat inspection services;
  • I will adhere to DAFM’s conflict of interest protocol;
  • andI am sufficiently fluent in both the spoken and written forms of the English language as may be required to deliver the Services.

I attach:

  • a medical certificate attesting to my physical ability to fully perform the Services required in a meat plant environment
  • and a tax clearance certificate.

Note:

Incomplete submissions or submissions containing false declarations may be excluded.

Submissions must include name, address, contact phone number, email address; details of qualifications and registration (or pending registration) with the Veterinary Council of Ireland; a medical certificate attesting to the Contractor’s physical ability to fully perform the Services in a meat plant environment; and a tax clearance certificate.

They must also contain:

  • indication of knowledge and understanding of the Hygiene Package;
  • an undertaking of willingness to comply with all Health and Safety requirements;
  • a general indication of availability;
  • a commitment to comply with all reasonable DAFM directions as they relate to provision of ante mortem and post mortem meat inspection services;
  • a commitment to adhere to DAFM’s conflict of interest protocol;
  • and an indication of fluency in both the spoken and written forms of the English language as may be required to deliver the Services.

All submissions must be signed, dated and witnessed. DAFM reserves the right to verify any statement made and to require further information. DAFM reserves the right to exclude any submission that does not supply all the relevant information.
