All Preschool and School-Age Children Require Quarterly Progress Reports

Progress Report and Annual Reviews

Early Intervention requires a “3 Month Progress Report” and a “6Month Progress Report” for each child receiving early intervention services. These reports are due to Kidz Therapy 3 weeks prior to their actual due date. The 3 Month Progress Report requiresinformal testing (using age equivalent tests). The 6 Month Progress Report requires formal testing (using standard deviation tests).

All progress report due dates are calculated according to the IFSP period (not the authorization period). The initial meeting date (upper right corner of IFSP) is the start of the IFSP period. If the date is 6/1/09, the 3 Month Progress Report would be due in our office by 8/11/09 (3 weeks before 9/1/09); the 6 Month Progress Report would be due in our office by 11/10/09 (3 weeks before end of IFSP period 12/1/09). It is your responsibility to keep track of the Progress Report due dates and to submit them to Kidz Therapy 3 weeks prior to those due dates. All preschool and school-age children require quarterly progress reports. PS and SA students receiving summer services require a progress report byAugust 12, 2011. The following timetable will be utilized to submit reports so we are in compliance with federal legislation:

School-age students: Progress reports need to be in our office no later than 11/1/10, 1/13/11 and6/9/11. Summer progress reports are due one week prior to the last date of summer service.

Preschool students: Progress reports need to be in our office no later than 11/8/10, 1/20/11 and6/16/11. Summer progress reports are due one week prior to the last date of summer service.

All School-age Annual Review reports are due to our office byMarch 10, 2011. All Preschool Annual Review reports are due by March 17, 2011. Follow procedures outlined in your Policy and Procedure Manual. If you begin services with a child after January 1, 2011, your Annual Review report will be due by May 2, 2011. If a school district schedules an Annual Review before our due date, we will notify you immediately. Any Annual Reviews that are scheduled before our due date, are due in our office one week prior to the meeting date.

All School-age and Nassau Preschool Annual Review Reports must be uploaded to “Annual Reviews and Progress Reports” via our secure website.

All Suffolk Preschool Progress Reports and Annual Reviews must be uploaded to “Suffolk PS Annual Reviews” (see instructions attached).

IEP Direct

As you know, most districts require that we use IEP Direct 7. On our website you will find a list of districts using IEP Direct 7 as well as those districts who have given us online access. If we have online access, therapists must input their progress reports and Annual Review goals directly into the district’s IEP.
