PARTICIPATION AGREEMENT AND WAIVER OF CLAIMS
A-Flight Sports Inc. is a corporate entity that renders service of setting up league participation in various sporting events arranging for the use of fields of play, the hiring of officials in league play and establishment of league rules.
As a term and condition of being allowed to participate in the league, each team and each individual participating player must acknowledge and understand that A-Flight Sports Inc., it’s agents, servants and employees will not be responsible for loss or injury to any individual. A-Flight Sports Inc. will not be responsible for medical expenses of any kind resulting from loss or injury suffered in any league activity. Each and every player must execute the waiver and release contained below. Failure to do so will make a player ineligible for league play. The participation of an ineligible player in any game will result in a forfeit being assessed against the team for which the ineligible player participates. In addition, each captain agrees that he or she will ensure that every participating player will sign the waiver and release, that the team captain will not allow any player that has not signed a release or waiver to play in a league game and finally that he or she will indemnify and hold harmless A-Flight Sports Inc., it’s agents, servants and employees from liability or expense arising through or out of the failure of every player to execute the waiver and release.
WAIVER AND RELEASE
In consideration for being allowed to participate and play in A-Flight Sports Inc. 2009 Softball League, as a member of (team name) , I do remise and release A-Flight Sports Inc. it’s agents, servants and employees from any and all claims, debts, duties, liabilities and obligations arising through or out of my participation in league sponsered events.

AGREEMENT & INDEMNIFICATION BY TEAM CAPTAIN
I am(insert name) the captain of the (team name)softball team and hereby agree and represent that I will not allow the participation of any player in any league game or activity who has not executed the Participation Agreement and Waiver of Claim form. In the event that an ineligible player (one who has not executed the Participation Agreement & Waiver of Claims form) brings any claim against A-Flight Sports Inc., it’s agents, servants and employees, I agree to indemnify and hold harmless A-Flight Sports Inc. and it’s servants and employees for any expenses incurred in connection with such claim including attorney’s fees and any amounts paid on account of such claim.
