Human Resource Data Element Definitions

School District/BOCES Code-- A unique code assigned to a district by CDE. Refer to School District/BOCES Code table.

Personal Information

Social Security Number--A unique number assigned to an individual.

EDID (Educator Identification)--A unique number assigned by CDE for an individual.

First Name--A name given to an individual at birth, baptism, or during another naming ceremony, or through legal change.

Middle Name--A secondary name given to an individual at birth, baptism, or during another naming ceremony, or through legal change.

Last/Surname--The name borne in common by members of a family. If possible, include title.

Ethnicity: Hispanic or Latino --a person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race. The term “Spanish origin” can be used in addition to “Hispanic/Latino or Latino.”

Federal Race/Ethnicity Reporting Category --the single category used to classify the student or staff member in aggregated federal or state reporting such as pupil counts, graduation rates, and assessment results. This designation is based on the race and ethnicity information provided to the reporting district by the student (or his or her parent/guardian) or staff member.

Highest Level of Education Completed-- The extent of formal instruction an individual has received (i.e., the highest grade in school or its equivalent or the highest degree received).

Required if Job Classification Code is 105, 106, 201, 204, 206, 216; as well as those staff with job class codes of 104 or 107 and assigned to a specific school (not 9980-districtwide). Not required if 07 - PTP Self Assessment is reported as Technology Assessment Type Used. If not required, zero-fill.
