Human Resource Data Element Definitions
School District/BOCES Code-- A unique code assigned to a district by CDE. Refer to School District/BOCES Code table.
Personal Information
Social Security Number--A unique number assigned to an individual.
EDID (Educator Identification)--A unique number assigned by CDE for an individual.
First Name--A name given to an individual at birth, baptism, or during another naming ceremony, or through legal change.
Middle Name--A secondary name given to an individual at birth, baptism, or during another naming ceremony, or through legal change.
Last/Surname--The name borne in common by members of a family. If possible, include title.
Ethnicity: Hispanic or Latino --a person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race. The term “Spanish origin” can be used in addition to “Hispanic/Latino or Latino.”

This designation is based on the race and ethnicity information provided to the reporting district by the student (or his or her parent/guardian) or staff member.

Not required if 07 - PTP Self Assessment is reported as Technology Assessment Type Used. If not required, zero-fill.
