2010 OHA Coach of the Year Nomination Form

NOMINATION FORM

CRITERIA:

Open to any coach who has been responsible for coaching a football team(s);

 Nominees will be judged for their overall performance and service to amateur football in BC;

 Nominees will be considered on the basis of coaching amateur performance only;

 Eligible coaches are those who have resided or whose primary coaching responsibility was in BC during the 2016 calendar year;

 Nominations may be submitted by any Football BC member organization;

 Organizations may submit only one nominee per category;

 Fully complete nominations forms and citations must be received by December 31, 2016;

 Nominations WILL NOT be accepted verbally or by phone;

Nominations should be typed and one-page in length;

 Orange Helmet Awards Committee reserves the right to seek nominations between the nomination deadline and the selection date, and the right to decline nominations;

Supporting Information: Please provide other comments, facts and figures for the Selection Committee to consider. Only one double-sided page (two pages if submitted electronically) in single-spaced 10 point font (or larger) of information will be accepted. Newspaper clippings are acceptable as part of the additional page of information. Please also make the effort to have video of the nominee available. Video does not need to be sent in with the supporting information, but should be available if the nominee is selected.
