Thank you for your interest in serving as a member of the ASHHRA Board of Directors. Please take a few minutes to review the board member job descriptions on the ASHHRA Website to learn about the eligibility requirements for each position (

To be considered as a candidate, this form must be filled out completely with a copy of your resume and a personal statement (see below). Members may submit information for more than one position provided that they meet the eligibility requirements for each position. Please send in separate forms for each position.

All of these materials must be sent by fax or e-mail to the Nominating Committee chair by close of business on Monday, Oct. 23, 2006. The contact information is provided at the end of this form.

Candidates will be slated by Nov. 22, 2006, and the complete slate will be sent to all ASHHRA members for ratification at that time. Final results will be announced on Dec. 23, 2006.
