Working With Your Health Care Team

A team approach to diabetes care integrates the skills of several health care professionals with those of the person with diabetes and family members into a comprehensive diabetes management program. The person with diabetes is the central team member because that person carries out most of the diabetes care recommendations on a day-to-day basis. Therefore, people with diabetes need to be able to effectively work with members of their health care team. Evidence shows that a team approach can facilitate diabetes management and lower the risk for complications from chronic disease.

What You Can Do:

Use the Working With Your Health Care Teampresentation and the supporting materials to help your employees learn how to best manage their diabetes. This is part of an eight-lesson series that provides up-to-date diabetes information and tools for your employees. Offer Understanding Diabetes first and the remaining seven presentations can be offered in any order. Reviews your company’s health plan’s benefits for dental, vision, and podiatric care.

Encourage your employees with diabetes to ask their health care provider for a referral to a diabetes self-management education program, preferably an American Diabetes Association Recognized Education Program or a program accredited by the American Association of Diabetes Educators.
