Dear WESS Customer,
An updated version of WESS 3.2.0 was released on Wednesday, 13 Feb.
Below is a description of the enhancements and improvements you will
find in this release. Also included are notes on the initial release
of WESS 3.2.0 and the previous WESS 3.1.7 release.
Many of the items are suggestions and defects identified and reported by
our customers. We continue to value your input. WESS Release Notes
Version 3.2.0 R4 – Release Date on Wed, 13 Feb 2013
Enhancements
WESS is changing the view and actions of the Select Subsystem page by
adding new buttons to assist in guiding users to the correct reporting
sections of WESS (artf118686).
The new buttons will allow users to better understand where to report
specific types of mishaps. The main subsystem page will now only have
four buttons as follows:
1. “Mishap/Hazard Reporting”
Select to report mishaps and hazards governed by OPNAV 5102.1:
- Shore/Ground, Afloat and Diving mishaps
- PMV/GMV, Tactical, and Off-Road Vehicle mishaps
- Recreational/Off-Duty mishaps
2. “WAMHRS” - (WESS Aviation Mishaps & Hazards Reporting System)
Select to report:
- Aviation mishaps and hazards governed by OPNAV 3750.6
3. “DJRS” - Dive Jump Reporting System
Select to log:
- On duty military diving operations for all DOD governed by
OPNAVI 3150.27
- On duty military personnel parachuting/cargo aerial delivery
operations for Navy and Marine Corps governed by OPNAVINST 4630.24
4. “Data Extraction” – (Previously labeled “Reports”)
Select to access safety data and generate preformatted reports (JASPER)
When the user selects the Mishap/Hazard Reporting button, eight new
buttons will appear as follows:
1. “Tactical Vehicle” – Select to report Tactical Vehicle mishaps:
- Tanks, self-propelled weapons, armored personnel carriers, amphibious
vehicles ashore, HMMWV, etc.
2. “Off-Road Vehicle Off-Duty” – Select to report Off-Road Vehicle
mishaps (off-duty):
- Recreational off-road vehicles while off-duty (e.g., 4-wheelers,
dirt bikes, dune buggies, etc.)
3. “PMV/GMV/GVO” – Previously labeled “Motor Vehicle Reporting System (MVRS).”
Select to report Private Motor Vehicle (excluding Off-Road Vehicle),
Government Motor Vehicle, Government Vehicle Other (excluding Tactical
Vehicle) mishaps:
- Passenger car, van, bus, ambulance, motorcycle, tractor-trailer,
construction tracked vehicle, forklift, road grader, agricultural-typewheeled tractor, aircraft tug, etc.
4. “Diving Ops On-Duty” – select to report On-Duty Diving mishaps:
- Diving operations occurring on-duty.
5. “Recreational Off-Duty” – select to report Recreational and/or Off-Duty
mishaps:
- Off-duty diving, swimming, boating, skiing, walking, bicycling,
go-cart driving, home improvements, fireworks, etc.
6. “Afloat On-Duty” –Afloat Mishap & Hazard Reporting System (AMHRS). Select
to report Afloat mishaps and hazards (Diving mishaps select ‘Dive Ops’ above):
- On board commissioned Navy or MSC vessels and embarked boats, landing
craft, or leased vessels.
- Loading, off-loading, or receiving services pier side.
- Amphibious/inshore warfare training operations up to the high water mark.
- Injuries to DoD personnel and non-DoD personnel on board.
7. "Shore/Ground On-Duty" (to include Shipyards) - Select to report Shore/Groundmishaps that include:
- DoD mishaps that occur on land, involve DoD operations, evolutions,
on-duty DoD personnel, and that results in injury or occupational
illness, or damage to private or DoD property.
- Injury or illness to non-DoD personnel caused by Shore/Ground DoD
operations, evolutions or on-duty DoD personnel.
- Parachute/Cargo Drop/HRST.
8. “WAMHRS, DJRS, Data Extraction” - Select to return to previous WESS Subsystems:
This will allow users to return to the WAMHRS, DJRS, and Data Extraction
selections.

5102 – Consolidated Specific Changes
1. An improved Afloat mishap reporting module (AMHRS) is available for the purposeof submitting mishaps that occur on or are involved with:
a. On board commissioned Navy or MSC vessels and embarked boats, landing
craft, or leased vessels
b. Loading, off-loading, or receiving services pier side
c. Amphibious/inshore warfare training operations up to the high water mark
d. Injuries to DoD personnel and non-DoD personnel on board.
2. The revised Afloat module contains the following improvements:
a. Approximately 50% reduction in data fields. Only data mandated by the
Office of the Secretary of Defense or data that is frequently requested bythe fleet or senior leadership has been included.
b. Improved screen flow. Enhanced organization coupled with data reduction has decreased the number of web pages to complete an Afloat report from about 70 pages to 13 or fewer.
c. Improved page-to-page performance. The Afloat module was modified to usetechnologies to optimize application speed to mitigate bandwidth limitations
afloat. Although some users may experience initial load times of up to
several minutes, once the initial load is complete, the page-to-page
performance in filling out an Afloat mishap report will be significantlyfast with users able to navigate from page-to-page with minimal lag time.
It is recommended that customers perform the initial load while pier sidewhen more bandwidth is available on any workstations that will be used toenter WESS report. Since the Afloat module is saved to your system memory, additional reports can be initiated without having to reload the module. Additionally, subsequent downloads, even while at sea, should be achievedat a faster rate.
Your browser settings will determine how long the Afloat module is retainedin your system memory. Customers may also access the Afloat module fastervia a direct link. The URL is . This link will take customers directly to the Afloat module and will bypassthe normal WESS Subsystem Selection page.
d. Improved control of report submission. A new releaser permission has been added to the already existing user (drafter) and notification permissions.
All Afloat Web Enabled Safety System (WESS) account holds will be drafters,and all users who have notification permissions will automatically becomereleasers.
This change will better control who is allowed to release data off the ship. As this will be a blanket account change, commands and Safety Authoritieswho have WESS accounts must review all users from their command to ensure the appropriate people have release authority.
e. Human Factors and Analysis Classification System (HFACS) incorporation.
Mishap causal factors have been completely overhauled. The revised Afloatmodule uses the DoD HFACS, which provides an enhanced method of analyzing,tracking, and trending the human element in mishaps.
f. Improved narratives. The revised Afloat module now contains separate areasfor event description, causal factor narratives, and recommendations.
g. Damaged Equipment. Damaged property will be submitted using the involvedproperty's Equipment Identification Code (EIC). EIC provides a consistent,reliable method of tracking, specific property that has been involved in amishap.
h. Community of Interest (COI) notification. Users with notification
permissions can view a redacted PDF report of mishaps released to a COI towhich they belong.
3. Private Motor Vehicle and Government Motor Vehicle mishaps (excluding tacticalvehicles) are reported through the Motor Vehicle Reporting System (MVRS). Aviation mishaps are reported using the WESS Aviation Mishaps and HazardsReporting System (WAMHRS) module. All other mishaps, including dive, off-dutyrecreational and on-duty shore mishaps should continue to be reported through the WESS system. Dive mishaps will be added to the Afloat module in a future update.
4. It is anticipated that most customers will have sufficient bandwidth to use therevised Afloat module. Customers who have zero bandwidth or who are unable to access the Afloat module for any other reason may report Afloat mishaps by NavalMessage.
5. The updates made in the revised Afloat module have rendered WESS-DS incompatiblefor reporting Afloat mishaps. Off-line submissions via WESS-DS for Afloatmishaps are no longer authorized.
6. A training presentation for the submission of an Afloat mishap has been placedon the Naval Safety Center webpage (
Documents/WESS/AfloatUserGuide.pptx). Afloat customers are encouraged to familiarize themselves with the improved reporting capabilities.
Version 3.1.7 - Release Data on Wed, 7 Nov 2012
Enhancements
1. The Jasper report interface was upgraded to a new version. The customer
interface has been slightly modified, but the functionality remains the same.
The export options are now displayed in a drop-down menu instead of individualicons for the different formats.
2. Added a notification feature which will display when Jasper is undergoing
maintenance and customers select to access the Reports feature or Pre-formattedReports (artf121958).
3750 - Aviation Specific changes
Enhancements
1. The WESS system will be converting to a new user's workspace in an upcomingrelease. During the transition the Notification folder in your Workspace willnot be populated; however, e-mail notifications will still be transmitted. Once the transition is complete notifications will once again populate and beaccessible from your WESS workspace (artf61496).
DJRS - Dive/Jump Reporting Specific Changes
Enhancements
1. Modified the dive log rejection email to display the comments entered by theindividual rejecting the dive log to communicate the reason for the rejection(artf34208).
2. Removed the ability for unit levels to set DJRS profiles to be Jumpers/Divers.Only those with profile creation ability will be able to designate DJRS profiles as either jumpers or divers (artf43179).
Bug Fixes
1. Corrected issue of dives showing in "Dives Pending Approving" with the approver shown as "awaiting approval from: unspecified" (artf62499).
2. Corrected issue of the "Complete" and "Delete" buttons not consistently
displaying for Draft reports (artf95085).
