COMMUNITY NURSES

The community nurses can be contacted on 020 82747088 or by leaving a message at the surgery.

HEALTH VISITORS

The health visitors are available to advise on children under the age of five and other related health matters. Phone direct on 020 8605 1916 or book an appointment with them on Wednesday afternoons at the surgery between 2.00 - 4.00pm.

All consultations are by appointment (some can be booked up to three weeks in advance and others only on the day). Saturday appointments are pre-bookable only during the week prior to the Saturday.

BOOKING APPOINTMENTS

Telephone: 020 8942 0066 between 8.30am - 12.00pm and 2.00pm - 6.00pm.All consultations are by appointment up to three weeks in advance or on the day except Saturday, which can be up to one week in advance but not on the day. These may be made by calling at or ringing the surgery or online (application forms in reception).

Telephone consultations are also available to be booked if you feel that you can discuss your problem with the doctor on the phone without needing to be seen at the surgery.We will endeavour to give you an appointment with the doctor of your choice, providing that the doctor is available that day.

If you are booking an appointment with a nurse (not available online due to complexity), please let the receptionist know the reason, so that you are given an appointment with the appropriate nurse.

CANCELLING APPOINTMENTS

If you are unable to keep your appointment please let us know as soon as possible. Appointments are in great demand and someone else could be seen in your place.

Appointments can be cancelled by calling us, online if you have registered for the online service or via our website

URGENT PROBLEMS

Please ring as soon after 8.30am as possible. If all available bookable appointments have gone you will be offered a triage phone consultation with a doctor. If your problem cannot be sorted out on the phone, the doctor will book you an appointment. For urgent problems arising later in the day, if all available bookable appointments have gone, you will be seen by whichever doctor is available and fitted in at the end of surgery. Such consultations will, by necessity, be brief to avoid long delays for other urgent cases.

HOME VISITS

These are for patients who are housebound or too ill to come to the surgery. Requests for visits should ideally be made before 10.30am. Please give a clear description of the problem. This will help the doctor to decide how soon to visit.

If the patient has a temperature or a rash, coming to the surgery will do them no harm and will not endanger others.

OUT OF HOURS

Between 6.30pm and 8.00am Monday to Thursday and from 6.30pm Friday to 8.00am Monday, including Saturday, Sunday and all bank holidays PLEASE CALL 111

If you need urgent advice or a visit, a doctor can always be contacted through the above number. Remember, this service is designed for genuinely urgent calls so please do not misuse it.

REPEAT PRESCRIPTIONS

If your doctor agrees, you can obtain repeat prescriptions for long-term medication. These can be obtained by written request on, preferably, your repeat reorder form or also online (forms from reception or our website)

Please give us at least two working days to fulfil a request. Do remember to allow for bank holidays. You may post a request with a stamped, self-addressed envelope. You can also use a pharmacy collection service - ask your pharmacy for details.

Please remember to tell us if you change your name, your address or especially your telephone number. Changes can be made using a form on our website

This practice operates within a strict boundary and if you move out of our area it will be necessary to register with a new doctor.

NEWLY REGISTERED PATIENTS

All newly registered patients are offered an appointment for a health check with our health care assistant, within three months of registering.

RESULTS OF TESTS

Please phone 020 8942 0066 between 11.00am - 12 noon or 4.00 - 6.00pm for results of tests you may have undergone. The telephone is not so busy then and the reception staff will be able to find the results quickly.

TELEPHONE ADVICE

If you need to speak to the doctor personally then please book a telephone consultation appointment or call at the end of the morning surgery, after 11.30am and before 12.30pm is the best time and you will be asked to leave a message for the doctor to call you back; phone 020 8942 0066.

CLINICS

ANTENATAL

Regular weekly clinics are run by the community midwives. The doctors are also very happy to share antenatal care with the hospital and appointments can be made with them at a time to suit you.

MINOR SURGERY

A full range of minor surgical procedures are performed at the surgery. Please discuss this with your doctor. We also have a wart clinic.

CHILD HEALTH DEVELOPMENT

We run a regular clinic on a Wednesday afternoon between 2.00 and 4.00pm both for immunisation and for baby health checks.

CERVICAL SMEARS

These are mainly undertaken by the practice nurses. Please let the receptionist know that you are making an appointment for a cervical smear.

DIABETES, RESPIRATORY, HEART DISEASE AND OTHER CLINICS

Patients with life-long diseases, e.g. diabetes, asthma, COPD or heart disease, need regular follow-up for these chronic diseases. Our practice nurses have been specifically trained in these areas and are running regular clinics to monitor these patients in order to keep them as active and healthy as possible. Please ask at reception when you book your appointment.

OTHER SERVICES

CHILD IMMUNISATIONS

The full range of immunisations is given. Reminders will be sent to you by the computer recall system. Please make a Baby Immunisation Clinic appointment on a Wednesday afternoon.

The current schedule (updated July 2016) given in surgery is:

The 5-in-one Diphtheria, Tetanus, Pertussis, Polio & Hib at 2, 3 and 4 monthsplus Pneumococcal and Men B at 2 and 4 months, and Rota-Virus (oral) at 2 and 3 months.

Hib and Meningitis C, MMR I, Men B and Pneumococcal (4 injections) at 12-13 months

The 4-in-oneDiphtheria, Tetanus, Pertussis Polio, pre-school booster and MMR booster at 3 years 4 months.

HPV vaccine for 12-13 girls only, the 3-in-one teenage booster and Men ACWY for 13-18years are given in school.

For your child's safety it is ESSENTIAL that you have your child properly immunised.

VACCINATIONS AND TRAVEL ADVICE

Routine vaccinations including those required for foreign travel are given together with general travel and malaria protection advice. Please complete a travel questionnaire (from reception or our website) before making an appointment with the practice nurse. (Please leave plenty of timebefore you travel). We are a designated Yellow Fever Vaccination Centre.

CONTRACEPTION

We are able to give advice and provide a full range of contraceptive services including long acting reversible contraception, coils and implants. Emergency contraception including the morning after pill is available each day. Please let the receptionist know if you need an appointment for emergency contraception.

BLOOD PRESSURE CHECKS

Blood pressure checks are carried out by our practice nurses and our health care assistants.

DRESSINGS AND REMOVAL OF STITCHES

Dressings and removal of stitches are carried out by our practice nurses.

YOUNG PEOPLE

We prefer children under 12 to be accompanied by an adult, preferably their parent, when making an appointment, although confidentiality is assured. Older children and young adults are assured of confidentiality and we are happy to see them alone if they choose.

NON-NHS EXAMINATIONS

Medical examinations for insurance companies, pre-employment etc are not undertaken in routine surgery; special arrangements can be made at the reception. A fee is payable – please see our notice board for list of fees.

NON-NHS CERTIFICATION

Certificates for private health insurance, holiday cancellation etc must be left for a minimum of 72 hours. A fee is payable for all non-NHS services – please see our notice board for list of fees.

SUGGESTIONS AND COMPLAINTS

We operate a practice-based complaints procedure, details of which are available on request. Complaints will be dealt with promptly and, we hope, amicably by our Practice Manager.

If you are unhappy with anything that has happened to you in the practice and would like to offer suggestions about how this may be put right, please discuss this with the Practice Manager. Alternatively, you may place suggestions in our suggestion box.

ACCESS FOR THE DISABLED

Our surgery is on three levels and has a lift and semi-automatic entrance doors activated with push pads or manually. An intercom is situated by the front door to alert staff to anyone requiring further assistance to enter the surgery.

PATIENT PARTICIPATION GROUP

Work with us to help develop our services and your healthcare experience by joining our Virtual Patient Participation Group. Forms are available at reception and on our website.

VOLUNTEER DRIVER SERVICE

Our volunteer driver service "Wheels Appeal" transport patients, who find it very difficult to get to the surgery on their own for their appointments. This is a fantastic service run by patients, using their own cars, to help their fellow patients. NEW DRIVERS ALWAYS NEEDED AND WELCOME if you would like to become a volunteer driver please ask at reception.

We regularly publish newsletters to keep our patients up to date with surgery and NHS related information. Make sure you receive your copy by signing up on our website

Copies are also available at the reception desk.

Patient Confidentiality and Data Protection

We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team.

However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

Zero Tolerance

We strongly support the NHS policy on zero tolerance. Anyone who abuses the GPs, staff or other patients whether verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.

INFECTION CONTROL

We are committed to the control of infection. If you have any concerns please speak to the Practice Manager.

Freedom of Information – Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available on request.
